**Location:** Montgomery County - King of Prussia, Plymouth Meeting, Norristown
**Hours/Shift:** M-F, 8:00-5:00, 1 weekend a month

Are you a dedicated and patient-focused Certified Nursing Assistant? Do you have a passion for providing compassionate care in a home setting? If so, we want you to join our team at Penn Medicine at Home (PMAH)!
**About the Role:** As a Home Health Aide, you will play a crucial role in implementing personalized care plans, assisting patients with daily activities, and providing support to their families. You will perform home health or hospice visits within a designated service area and ensure timely and accurate documentation of patient care.
Key Responsibilities:
Patient Care:
+ Assist with bathing, oral hygiene, and grooming.
+ Prepare light meals and help patients with eating.
+ Help patients with dressing and using the bathroom.
+ Empty Foley catheter drainage bags and record the amount.
Exercise and Transfers:
+ Support patients with home exercise programs.
+ Assist with walking and transferring from bed to chair.
+ Reposition patients in bed for comfort.
Patient Environment:
+ Perform household tasks essential to patient care.
+ Assist with pre-poured medications.
+ Provide respite care to relieve primary caregivers.
Documentation:
+ Accurately document care provided according to standards.
+ Complete assigned hours and communicate schedule changes.
+ Verify and submit all activity sheets and reports.
Communication:
+ Report any unusual findings or changes in patient status.
+ Facilitate communication between the family, RN Case Manager, and Scheduling Coordinator.
Plan of Care:
+ Follow the written care plan and provide care as outlined.
+ Review the biweekly care plan before each visit.
Employee Travel:
+ Provide your own transportation to patient homes and meet state vehicle requirements.
Education and Training:
+ Attend annual in-services and complete at least 12 hours of continuing education each year.
+ Understand and adhere to PMAH procedures and competencies.
Required Credentials:
+ Car Insurance
+ Certified Nurse Aid (CNA)
+ Certification as a Nursing Assistant (in PA) or Home Health Aide (in NJ), or be a nursing student in an accredited BSN program with a 3.0 GPA.
+ Driver's License
Education and Experience:
+ High School Diploma or GED
+ At least 1 year of recent experience as a Nursing Assistant or Home Health Aide
+ Graduate of a certified nursing assistant program or a nursing student in an accredited BSN program with a 3.0 GPA.
