    Nurse (Community Health)

     

    Fort Johnson (formerly Fort Polk) is an Army post located in Vernon Parish in Louisiana seven miles from Leesville, Louisiana. The city of Leesville, population 6700, is situated between Shreveport and Lake Charles. It is 63 miles to Lake Charles, Louisiana and 50 miles to Alexandria where the nearest municipal airport is located. Fort Johnson is the second largest employer in the state of Louisiana and has a huge economic impact on the entire state. The Army is situated on 198,000 acres. The Department of the Army actually owns 100,000 acres with the remaining acreage belonging to the United States Forest Service and part of the Kisatchie National Forest. The land that the post is on is both of historical and archeological value.

    Responsibilities

    Duties

    + Serve as Community Health Nurse, managing the installation program for Human Immunodeficiency Virus (HIV), Acquired Immune Deficiency Syndrome (AIDS), sexually transmitted infections (STI), tuberculosis, and other communicable diseases.

    + Provide education as requested or required in communicable diseases, administration of medications, and infection control.

    + Advise and develop action plans for the Family Advocacy Program Multidisciplinary Inclusion Action Team (MIAT) and Case Review Committee on pediatric health and developmental concerns.

    + Advise on and report communicable disease clusters or outbreaks.

    + Develop and provide educational and consultant services to members at risk for or concerned about HIV, STDs, tuberculosis, and hepatitis.

    + Screen patients’ medical history, travel itinerary, and other travel related factors to determine health risks.

    + Conduct Community Health Assessments of various communities within the Ft Johnson catchment area to include individual patients, family members, units, and installation organizations.

    + Contact tracing for communicable diseases.

    + Conduct Inspections and epidemiological investigations.

    + Daily specialty collaboration among peers, state/local officials, and community partners.

    + Ability to brief on multiple healthcare topics

    Qualifications

    CONDITIONS OF EMPLOYMENT & NOTES:

    1. Current Basic Life Support Certification.

    2. Is immune or is properly immunized against the communicable diseases as is required by OSHA standards for health care workers.

    3. This position is a Testing Designated Position (TDP) subject to applicant testing and random drug testing. Applicants tentatively selected for appointment to this position will participate in drug urinalysis testing, unless currently occupying a TDP within the Department of Defense. Tentatively selected applicants will be required to sign DA Form 5019 (Condition of Employment for Certain Civilian Positions Identified as Critical Under the Department of the Army Drug-Free Federal Workplace Program) requiring participation in random drug testing as a condition of employment. A job offer to an applicant who is a not a current employee will be withdrawn if the applicant refuses to be tested. Applicants with verified positive test results shall be refused employment. Applications from such individuals shall not be considered for employment for a period of 6 months from the date of the test results.
