    This is a hybrid role that will require you to go into the downtown Portland office 1-2 days a week.

    Job Summary

    This position provides technical assistance, coaching, and training to the CareOregon provider network in areas related to primary care medical home improvement and practice transformation or to the Medicare Advantage performance measures such as Stars, CAHPS, HCC coding and Risk re-capture. This position spends time designing and facilitating collaborative meetings and providing on-site support to member clinic leadership and care teams to implement systems, processes, and workflows that improve quality outcomes, patient experience, clinic and team satisfaction, and reduced health care costs. This position partners closely with CareOregon and CCO regional teams and leadership to develop, oversee, and implement programs, initiatives and payment models that support and enhance whole-person health. This role requires a high degree of collaboration, coordination, and influence with multiple internal and external stakeholders in positions of clinical and operational leadership.

    Essential Responsibilities

    Technical Assistance

     

    + Provide on-going on-site support to clinical and operational leadership teams and staff of in-network and non-network clinics in the areas of:

    + Primary Care focus - Patient-centered medical home leadership, data systems, team-based care, access, clinic support structures, and care management.

    + Medicare focus – Medicare Stars, CAHPS, and Risk coding, data systems, access, clinic support structures, and care management programs that support dual-eligible members with high needs.

    + Provide technical assistance to participating member clinics on the development of actionable data reports and applied use of reports within their settings (both visual management and electronic based systems).

    + Assist clinics in planning and reporting for alternative payment programs in areas related to patient-centered primary care and high-performing medical homes or to Medicare Stars, CAHPS, and Risk coding.

     

    Coaching & Training

     

    + Design and lead collaborative meetings of primary care learning cohorts involving clinic staff and leadership teams of external organizations as a trainer and facilitator.

    + Serve as an advanced trainer and facilitator. Apply adult learning principles; influence learner outcomes and establish change agents at the clinic level.

    + Assist in the development and support of local resources to enhance service delivery through train-the-trainer programs.

    + Coach and motivate member clinic teams on improvement projects, including development of project goals, dates, and timelines.

    + Develop and lead clinic team-building activities as needed. Train clinic leadership teams to provide these services to their staff as needed.

     

    Innovation

     

    + Research clinical approaches and best practices; provide strategic input and focus towards either CareOregon Primary Care or Medicare teams and CCO regional and clinical leadership.

    + Collaborate with CareOregon Primary Care or Medicare leadership in developing, defining, and conceptualizing new joint ventures among the primary care and CCO networks that will ultimately result in innovative or more effective services.

    + Lead or participate in creating and implementing evaluation systems and tools for key process and outcome measures in the clinic setting.

    + Partner with clinics in identifying opportunities for programmatic improvement.

     

    Project Management

     

    + Lead a variety of small to complex improvement projects relating to primary care medical home development and optimization with network partners; projects may span across multiple geographies.

    + Using project management knowledge, identify appropriate teams, including internal staff, as well as external partners, member clinics, and community groups.

    + Compile assessment information, document needs, and create work or project plans.

    + Apply appropriate improvement tools and technology essential to meeting project goals and communicate with other departments as directed.

    + Establish and ensure effective channels of communication for reporting on project status, coordinating changes, and making decisions.

    + Collaborate with others to ensure the coordination of development and planning efforts between multiple external community organizations and internal departments.

    Experience and/or Education

    Required

    + Minimum 6 years’ experience in healthcare (or 4 years’ experience with a college degree in Healthcare Administration, Public Health, or related degree); experience should include at least 2 years’ experience in a primary care setting and/or direct patient care and 1 year experience coaching, guiding, and/or facilitating health care professionals in quality improvement and/or process improvement practice

    + Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle

     

    Preferred

     

    + Experience with a variety of common electronic medical records (e.g., EPIC, Athena, NextGen, Cerner, Centricity, eClinical Work, AllScrips, e-MDs, and Practice Fusion)

     

    Knowledge, Skills and Abilities Required

     

    Knowledge

     

    + Advanced and applied knowledge in areas related to the primary care medical home or Medicare

    + Thorough and applied knowledge of clinic operations and charting systems

    + Applied knowledge and experience with quality improvement tools and techniques in health care or related setting including quality measurement and improvement methods and ability to coach and partner with network providers on the use of these tools and techniques.

     

    Skills and Abilities

     

    + Ability to effectively identify deficiencies or delivery problems, and in partnership with network providers develop action plans and implement solutions

    + Ability to provide advanced support for providers/clinic staff regarding documentation, workflows, and dot phrases or templates to improve efficiencies

    + Ability to troubleshoot with clinical staff on how to pull data reports

    + Ability to provide advanced functional and technical input to CareOregon teams and external partner organizations to advance and support CareOregon programs, goals, objectives, and outcomes

    + Ability to lead projects and work plans

    + Ability to represent the organization in public and private meetings and other gatherings as a subject matter expert in multiple areas of specialty

    + Advanced computer application skills including with MS Windows, Word, Excel, and Outlook with proficiency using Power Point, Access, Visio and internet browsers/search functions

    + Strong communication skills, including active listening, verbal, and written

    + Strong relational skills

    + Ability to influence and effectively coach and lead clinical and operational leadership and clinical teams of community groups that have distinct charters, missions, and goals in areas of practice transformation and the primary care medical home

    + Ability to work in a community (internal and external) with diverse individuals and groups to maintain cooperative and productive working relationships with department management team, internal and external customers

    + Ability to effectively collaborate with and lead colleagues and multiple internal and external groups

    + Ability to gain cooperation and support through discussion and persuasion

    + Effective skills in facilitating teams/groups, organization dynamics, and consensus building

    + Ability to coach and clearly articulate project/initiative/programmatic requirements, recommendations, and best practices to network partners at all levels of their organization

    + Demonstrated skill in conveying appropriate level of detail effectively to multiple audiences

    + Demonstrated strength in the areas of critical analysis, quantitative reasoning, and creative problem-solving skill

    + Excellent organizational skills and the ability to manage multiple tasks from multiple sources
