    Children 1st is the single point of entry for all DPH Child Health programs and services for children, birth - 5 years old, and links eligible children to early intervention services, as well as other public health programs and community-based resources. The Children 1st Program Support Coordinator will be responsible for providing support for and coordination of Children 1st program services, referrals, contracts, and data submissions.

     

    + Provides support and coordination for Children 1st.

    + Provides technical assistance, training and referral support to Children 1st program staff, providers, and partners in public health districts and partnering agencies.

    + Responsible for intake and routing of referrals to Child Health programs.

    + Provides outreach to families with identified risk factors.

    + Provides ongoing quality assurance and improvement for program data collection and submission.

    + Responsible for supporting state and federal requirements and guidelines.

    + Provides ongoing coordination and monitoring to support program timelines, reports, contract requirements are completed as required.

    + Responsible for effective communication with other DPH Programs, families, public health districts, agencies, partners, and referring providers.

    Preferred Qualifications:

    Bachelor’s degree in a related field from an accredited college or university AND 2 -4 years related experience.

     

    High school diploma/GED and four (4) years of professional level experience related to the area of assignment.
