Ebenezer Ridges Care Center highlights a full continuum of care campus including Transitional Care, Adult Day Programs, Intergenerational Child Care, Long Term Care, Enhanced Care and Assisted Living/Memory Care. We are located across the street from Fairview Ridges Hospital at the I-35E and I-35W split in Burnsville, MN. We are seeking a LPN- Licensed Practical Nurse to support our residents in our Skilled Nursing Facility (Transitional Care) in Burnsville, MN! This role performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures.

This Nursing schedule includes;

+ 32 hours every two weeks

+ Day shift

+ Includes every other weekend Saturday/Sunday and holiday rotations

Responsibilities:

+ Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practices

+ Lifts and transfer residents as needed, Initiates emergency treatment as needed

+ Monitors work and performance of unlicensed staff

+ Updates RN with current observation and completes documentation of resident health status

+ Communicates changes in residents conditions to relevant individuals

+ May support RN by completing resident assessments, service agreements and care plans

Required:

+ MN LPN licensure

+ AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers

Preferred:

+ Able to work independently

+ Good communication and interpersonal skills

+ Previous healthcare experience

+ Comfortable using computers, hand held devices, phones and fax machine
