Job Description:
The Community Health Manager- Grants works to support the development and implementation of initiatives that improve and strengthen the education system. This position focuses on supporting the Intermountain Community Care Foundation (Foundation) and its grantees in K-12 education innovation initiatives and other projects as assigned. The Community Health Manager- Grants builds relationships to advance education initiatives and support grantees and collaborators in implementing innovative educational programs and strategies.
Position Details:
This role will work Monday-Friday, during regular business hours. This is a hybrid position, with in-person meetings required. Incumbent must reside in Utah.
Essential Functions
+ Identifies and engages new organizations to receive grant funding and implement the Foundation’s education innovation initiatives.
+ Manages a portfolio of assigned grants, review progress reports, and communicate frequently with grantees and collaborators to monitor progress on project work plans.
+ Build strong working relationships with the Utah Department of Education, local school districts, educational institutions, and other key constituents to advance education innovation initiatives and the Foundation’s strategic priorities.
+ Engages with new and established community organizations to better understand their strengths and challenges, represent the Foundation’s strategies accurately, and identify opportunities for the Foundation to respond effectively.
+ Maintains current knowledge on education issues important to the Foundation; conduct literature reviews, establish a network of professional contacts, and participate in relevant conferences and other educational programs.
+ Conducts programmatic research and “bright spotting” of programs relevant to Foundation priorities, compile information and data to inform the development, implementation, and evaluation of Foundation priorities and programs.
+ Assist in evaluating Foundation initiatives; develop appropriate metrics and measurement systems, collect data, compile reports, and streamline measurements across initiatives.

Required Qualifications
+ Demonstrated success in a role supporting educational program development and transformation.
+ Demonstrated experience with policy, systems, and programmatic approaches to education innovation.
+ Demonstrates outstanding interpersonal skills, ability to establish trust and rapport with diverse stakeholders, and talent for forming and leading collaborations.
+ Ability to collaborate and contribute to a supportive team environment.
+ Proficiency in PowerPoint, Word, Excel, and Outlook.
+ Proven ability to design and implement programs leveraging community development finance tools.
+ Experience working in or collaborating with educational institutions and organizations
Preferred Qualifications
+ Seven (7) years of relevant professional experience.
+ Bachelor’s degree in education, education administration, public policy, or related area from an accredited institution. Education is verified.
+ Demonstrated experience partnering with community development organizations.
+ Experience with education systems.

+ Ongoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
+ Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
+ Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
+ For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
