    Job Description:

    The overarching goal of this position is to optimize the performance of Highland Family Medicine (HFM) and Geriatric and Medicine Associates (GAMA) by leading improvement in operational efficiency, financial performance, and staff engagement. This position requires a developed understanding of practice operations, Epic, revenue cycle management, and performance improvement methods. Also essential is the ability to direct multi-faceted projects across various settings, departments, and programs. Strong leadership and consensus-building skills are essential. This position directs and provides clinical oversight for the Care Management and Population Health Teams, oversees value-based contract projects, and acts as a project manager to pilot new clinical and operational initiatives with each of the department’s mission areas (Education, Research, Clinical, Operational, and Financial)

     

    Additional functions include building and maintaining a culture of creative problem solving, ownership, diversity and inclusion that enables innovation, and empowers team members with responsibility and accountability. This role provides direct clinical and administrative supervision for the HFM and GAMA care management team, and directs practice transformation initiatives at HFM and GAMA according to the tenets of NCQA’s Patient Centered Medical Home, the Health Equity Program Support Office (HEPSO), managed care plan incentives and other applicable initiatives and awards related to value based payment contracts. This position supports patient centered care and care coordination at HFM and GAMA through development and implementation of new processes, policies, and programs to increase quality of care and efficiency of service provision to patients by guiding processes, coaching clinicians and staff, and supervising employees in order to improve health care outcomes, reduce disparities, and generate revenue for the department.

    Salary Range:

    $98,500 - $125,000

     

    _The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._

    Job Requirements:

    **Qualifications** :

    **Education** : Bachelor’s of Science in Nursing required; Master’s Degree preferred.

    **Experience** : Minimum 5 years previous management experience in clinical ambulatory operations. Clinical care management experience required. Experience managing continuous quality improvement initiatives required.

    **License/Certification Required** : License to practice as an RN in NYS.

    **Skills** : Superb organizational and management skills with ability to carry out all aspects of job independently. Proficiency with Epic, Microsoft Outlook, and Excel required. History of demonstrated efficiency in prioritizing assignments, skill in proactively resolving problems and recommending and implementing continuous quality improvement projects.

     

    Demonstrated capacity to frame complex situations and present options.

     

    Effective written and oral communication skills.

     

    Ability to provide guidance to people with a wide range of cultural backgrounds, training and experience

     

    Enjoy working independently and as part of a team.
