Stony Brook University School of Medicine of the State University of New York invites applications for the position of Nurse Practitioner or Physician Assistant in the division of Endocrinology.
The Nurse Practitioner or Physician Assistant will see and treat patients with diabetes admitted to Stony Brook University Hospital and will demonstrate skills in collaboration, teamwork and problem solving to achieve desired goals.
+ Independently assess and evaluate new and follow-up consults to the inpatient diabetes consultation service at Stony Brook University Hospital.
+ Perform daily physical exams of patients with diabetes with special attention to body systems that may be affected by diabetes.
+ Review 24-hour tests of blood glucose values and recommend daily orders for medication to primary clinical team.
+ Educate patients and family members regarding diabetes self-management principles based on recognized curricula and national standards of care.
+ Identify discharge needs at the point of initial patient contact and assist in the transition of care planning.
+ Rotate through the established “call “schedule of the diabetes service and carry a paging device.
+ Demonstrate skills in collaboration, teamwork and problem solving to achieve goals.
+ Knowledge of basic software packages and hospital information systems (EMR).
**SPECIAL NOTES:** This position requires 8-hour shifts and includes rotational coverage of weekends and holidays.
Qualifications
REQUIRED QUALIFICATIONS** **:
**_Nurse Practitioner:_** NYS license and certification as a Nurse Practitioner. Master’s degree (foreign equivalent or higher) in nursing or completion of a course for a Nurse Practitioner.
**_Physician Assistant:_** NYS license and certification as a Physician Assistant. Bachelor’s degree (foreign equivalent or higher) from an accredited school of Physician Assistants.
_Must be eligible to sit for and pass the exam for_ _Certified Diabetes Care and Education Specialist (CDCES)_ _within 1 year of start date._
PREFERRED QUALIFICATIONS:
Certified Diabetes Care and Education Specialist (CDCES). Three to five years acute care as a NP/PA experience in diabetes management and education in a hospital setting. Knowledge of current clinical guidelines and standards from the American Diabetes Association, American Association of Clinical Endocrinologists and American Association of Diabetes Educators.
