    Our Position: We are currently seeking a Coordinator of Community Supports to provide oversight to our Community Habilitation program. This position also involves:

     

    + Providing direct oversight to managers and direct service professionals assigned to the Community Habilitation program.

    + Assisting the Director in tracking and data entry, as well as providing oversight of delivery of services and supports.

    + Ensuring that programs meet regulatory requirements related to OPWDD and/or other governing bodies.

    + Managing oversight of fiscal regulatory information and of training, hiring, and evaluation of staff.

     

    Our Rate of Pay: $25.00 - $29.00 per hour

    Our Requirements:

    + Master's Degree in a Human Service field, including but not limited to: Sociology, Special Education, Rehabilitation Counseling and Psychology PLUS one year of direct service experience with the developmentally disabled.

    OR

    Bachelor's Degree in a Human Service field, including but not limited to: Sociology, Special Education, Rehabilitation Counseling and Psychology PLUS three years of direct service experience with the developmentally disabled.

    PLUS

    Two years of supervisory experience.

    Position Summary:

    Provides oversight to Community Support & Services programs (e.g. respite, community habilitation, Family Supports & Services programs, Individual Supports & Services, ECDMH programs, etc). Provides direct oversight to managers and direct service professionals assigned to these programs. Responsible to assist the Director in tracking and data entry. Responsible for the oversight of delivery of services and supports. Ensures that programs meet regulatory requirements related to OPWDD, OMH, Medicaid and/or other governing bodies. Manages oversight of fiscal regulatory information and of training, hiring and evaluation of staff.

    Reporting Relationship:

    Reports directly to the Director of Community Supports & Habilitation Services.

    Job Duties and Responsibilities:

    + Oversees direct care services to people in areas such as, but not limited to, activities of daily living, money management, recreation activities, parenting skills, and sexuality training.

    + Advises managers and staff on how to intervene with peoples inappropriate and/or potentially injurious behaviors in the least restrictive manner possible.

    + Attends intake meetings and ensures necessary documentation is obtained from the Care Coordinator/Intake Department prior to distribution of cases.

    + Ensures individuals’ overall safety through maintaining safety standards, familiarity with emergency safety procedures to the best possible extent. Ensures a clean safe environment by regular environmental quality assurance checks.

    + Monitors Life Plans to ensure such plans are consistent with the person’s needs, preferences, and personal goals to make them to live, work, socialize, and participate in the community; Tracks individual staff action plans to ensure they are completed in a timely fashion; Ensures that all plans address individual participants overnight needs/safe guards consistent with the Life Plan.

    + Reviews monthly progress notes for participants to ensure that they reflect the person’s response to services provided and to ensure compliance with OPWDD requirements; Tracks to ensure all progress notes are done contemporaneously to service provision.

    + Monitors and tracks individual Life Plans and Level of Care Eligibility (LCED) to ensure maximization of waiver funding for habilitation services. Monitors and assists Program Managers with OPWDD requirements as they arise.

    + Monitors implementation of service plans through records review, regular supervision and consultation; as needed participates in case conferences; ensures regular reassessment of Staff Action Plans; ensures plans and monthly notes are completed in appropriate time frames.

    + Implements programs to assist individuals to obtain the most integrated level of services consistent with their desires and program plans.

    + Ensures maximization of service units through completion of weekly billing

    + Reviews & increases enrollment of program to meet revenue and annual budget goals.

    + Assists in the evaluation of program effectiveness from previously stated goals, objectives and criteria and makes recommendations to the Director.

    + Ensures that all statistical and service records are adequate, accurate and up-to-date, and that required reports are forwarded to the Director.

    + Facilitates intradepartmental communication to promote effective provision of services for all program participants

    + Ensures that all staff has received needed training. Ensures training is updated as required in both mandatory and optional areas.

    + Advises Director of program progress, efficiencies, needs and ensures reports are completed in a timely fashion.

    + Ensures that Director is informed of staffing needs, performance levels and disciplinary actions required.

    + Ensures individuals safety when participating in community activities.

    + Updates and tracks weekly metrics to ensure department annual fiscal goals are being met.

    + Evaluates program dates and progress through reporting utilization.

    + Provides staff with regular supervision to ensure delivery of stated services.

    + Monitors all required documentation for individual and agency records to ensure accuracy. Oversees maintenance of those records as appropriate.

    + Coordinates the development of intervention/training objectives. Implements, documents and monitors the progress of required individual plans.

    + Ensures that staff update and monitor the program specific plan as needed but at least every six months.

    + Monitors and/or assists in the assessment of the individuals’ skills and behavior.

    + Evaluates program data and progress.

    + Coordinates regular staff meetings and trainings as well as attend supervisory conferences and other assigned meetings.

    + Responds to emergency phone calls as part of the Agency’s emergency contact procedure.

    + Completes routine documentation responsibilities and other duties as assigned by the Director.

    + Ensures integration of the program with other services offered by the agency movement of individuals to Waiver/Medicaid based programs from County/non-Waiver/non-Medicaid programs.

    + Acts as a liaison with community organizations and program sites.

    + Assists with the assessment and eligibility determination of program participants.

    + Ensures compliance with OPWDD and Medicaid regulatory needs.

    + Ongoing comparison of all approved grants/budgets versus actual expenses identifying any areas of concern.

    + Complete purchase orders and check requests, assuring purchases are within budget.

    + Maintain records of invoices, receipts and expenses.

    + Have a thorough understanding of all policies and procedures relating programming

    + Review internal procedures and make recommendations for improvement.

    + Performs any other duties as assigned.

    Qualifications and Education Requirements:

    Master’s Degree in a Human Service field, including but not limited to: Sociology, Special Education, Rehabilitation Counseling and Psychology PLUS one year of direct service experience with the developmentally disabled.

    OR

    Bachelor’s Degree in a Human Service field, including but not limited to: Sociology, Special Education, Rehabilitation Counseling and Psychology PLUS three years of direct service experience with the developmentally disabled.

    Two years supervisory experience.

Receive supervision instruction in a positive manner. Adhere to and administer Agency’s policies, procedures, rules and regulations. Maintain a professional attitude and appearance to at all times.

     

    + Equipment Operation:

     

    The ability to operate business equipment; telephones, fax machines, computer hardware and software, including applications and programming, copiers, calculators, etc.
