The CAPABLE Care + Connect program seeks a **_Research Study_** **_Occupational Therapist_** (OT) with older adult experience to join our research study team. The research study seeks to adapt the existing CAPABLE program for older adults who are in home-based primary care (JHOME) who are at risk for or experiencing social isolation. This role will work under the direction of the research study Principal Investigator (Dr. Thomas Cudjoe), in collaboration with the Lead CAPABLE Registered Nurse.
The OT will implement the CAPABLE Care + Connect study protocol after completion of the required training through Johns Hopkins University School of Nursing CAPABLE on-line training. The CAPABLE Care + Connect program consists of up to six OT home visits over 4–5 months. The OT will work with CAPABLE Care + Connect participants to understand goals and assess and implement strategies. The OT meets with participants in the family’s home. The OT uses training and motivational interviewing techniques to conduct a comprehensive interview to identify and prioritize issues that matter to the participants. These goals become the foundation for brainstorming and action plan development and implementation. The OT documents the participants’ own goals in up to 3 Action Plans and creates a Work Order for the Handy Worker.
The OT interacts and communicates with the CAPABLE Care + Connect Nurse (RN) and other team members as needed to ensure close collaboration throughout the research study.
Specific Duties & Responsibilities
+ Conduct a comprehensive, semi-structured interview and home environmental assessment to identify issues related to ADLs, IADLs, home safety, independence, and other topics as prioritized by the older adult.
+ Work with the participant to observe functional performance for safety, efficiency, and difficulty. Also, assess the presence of environmental barriers for each targeted area.
+ Based on participant identified goals, develop a prioritized work order to include home modifications, repairs, and necessary items that will help the participant meet their goals. This includes communicating with the handy worker.
+ Engage participants in problem-solving to identify behavioral and environmental contributors that may impact function.
+ Coach and guide the participant in building self-efficacy and confidence.
+ Reinforce brainstorming techniques, strategic thinking, and more independent problem-solving to promote the ability to live at home and in the community.
+ Provide education and resources to address future needs and the participant’s priorities.
+ Maintain and manage equipment used by study participants (inventory, order, transport) and direct/monitor handy worker activities according to the work orders.
+ Communicate with the RN, Handy Worker, and other team members on a regular basis (team meetings every 2 weeks; weekly updates via email).
+ Document visits timely and thoroughly in database system.
Minimum Qualifications
+ Bachelor's Degree in OT or related discipline.
+ Current licensure from MD Board of Occupational Therapy.
+ One year experience as a licensed OT required
+ Additional education may substitute for required experience and additional related experience may substitute for required education beyond HS Diploma/Graduation Equivalent, to the extent permitted by the JHU equivalency formula.
Preferred Qualifications
+ Experience conducting home environmental assessments.
+ Experience working with older adults in the home setting.
+ Ability to work from a collaborative, client-centered model.
+ Willingness and ability to implement a protocol and balance with clinical judgment.
+ Strong organizational and communication skills.
+ Able to commute to Baltimore/surrounding county areas to participant homes.
