    Responsibilities:

    GENERAL PURPOSE:

    Under guidance from the CSP-FLR Program Manager and with latitude for independent judgment, the Health Educator II will be responsible for creating and implementing the public education, training and outreach (PETO) plan for the Cancer Services Program of the Finger Lakes Region. The position will coordinate all outreach and in-reach activities providing education and training to partners, providers and community members to reach the populations of focus. This position has some programmatic and strategic responsibility to implement the work plan and oversee the processes to ensure service, quality and outcomes to increase cancer screenings. The position will serve on community coalitions to build relationships that will propel the efforts of the CSP-FLR in the communities it serves and will function in multiple capacities to assist CSP-FLR in attaining all performance measures and program goals. This position works primarily in Wayne, Ontario, Seneca and Yates Counties. Work is also conducted in Monroe and Livingston Counties.

    ESSENTIAL FUNCTIONS

    Community Outreach

     

    In collaboration with the Program Manager, design, manage and deploy activities of the CSP-FLR. The position will lead educational and outreach activities to both CSP-eligible individuals and identified populations of focus to recruit and enroll eligible uninsured and underinsured individuals in need of breast/chest, cervical and colorectal cancer screenings. Assist in the planning and coordination of community partnership meetings, cancer awareness events, and public relations campaigns to raise awareness of cancer prevention and screening. Develop outreach plans to reach clients. Monitor metrics and evaluation processes, analyze and track results and trends for client satisfaction and referral patterns, maintain compliance with HIPPA and PHI management.

     

    Community Partners

     

    Educate community partners about the importance of screening by attending community health-related talks and events, and networking with those in attendance to increase awareness of the CSP-FLR; participate on community coalitions as assigned, and bring back information that would be helpful to the CSP-FLR team. Establish and maintain relationships with individuals and community-based organizations for partnership opportunities between the CSP-FLR and the communities it serves. Attend CSP-FLR, CCHP and partner-sponsored events.

     

    Provider Relations

     

    Responsible for coordination between providers and CSP-FLR to ensure protocols are followed for quality service delivery. Conducts periodic provider check-ins to replenish CSP-FLR materials, collect needed documentation and reports and ensure familiarity with CSP-FLR program. Maintain updated and accurate records of provider services and contact information. Coordinate screening events between providers and CSP-FLR to ensure all eligible patients are recruited, processed, tracked and screened appropriately.

     

    Reports and Recommendations

     

    Maintain a working knowledge of cancer prevention and screenings for all CSP-FLR covered cancers (breast/chest, cervical and colorectal) including screening eligibility and guidelines. Stay abreast of the literature regarding cancer prevention and attend pertinent meetings and seminars. Prepare analytical reports and recommendations for program enhancements.

     

    Program Support

     

    Assist co-workers as needed by answering phones, assisting with follow up calls, assuming other duties when coworkers are out of office for sickness or vacation. Enroll eligible clients into the program for cancer screenings. Follow up with patients for appointment reminders, recall and consents. Aids in provider credentialing process.

     

    Other projects and job duties as assigned.

    QUALIFICATIONS

    REQUIRED:

    + Bachelor's degree

    + 2 years of related experience or equivalent combination of education and experience

    + Excellent communication skills; the ability to interact effectively with a wide range of individuals, including clients, peers, clinicians/researchers at all levels, and staff in a variety of office settings; ability to work independently and as part of a team; ability to work effectively with people of different cultural and economic backgrounds.

    + Advanced knowledge of Microsoft Office Suite, internet search techniques, and basic knowledge of medical terminology and ability to learn and master new software quickly.

    + Valid NYS Driver's License and reliable transportation.

    + Ability and willingness to travel regionally throughout a six-county area on a weekly basis.

    + Able to work during weekends and evenings

    + This position primarily works in Wayne, Ontario, Yates and Seneca Counties. A satellite office is in Canandaigua, NY. Travel to the main office in Rochester is required as needed. Work may also take place in Monroe and Livingston Counties.
