    Position Title: Enrollment Coordinator

     

    Reports to: Intake Manager

     

    Job Summary: The Home Care Enrollment Coordinator works with the patients, referrals, and Community Outreach Representatives to help facilitate setting up services for new and existing clients to receive homecare. He/she is responsible for processing daily referrals received from sources such as hospitals, nursing homes, community physicians, online campaigns and also assist patients with eligibility and guide them through all service options to receive the best care based on their needs.

    Responsibilities include:

    + Consistently maintain a caseload of potential clients and follow up with them on a continued basis

    + Be the liaison between the contracts and clients to get them eligible for services

    + Schedule nursing assessments as needed to assess patients in their homes

    + Screen patients to find out the best level of care; obtain all required paperwork

    + Manage the referral intake process from initiation through the prospect stage; generate daily referrals through the E-Pace system

    + Manage the eligibility process of patients as they enter True Care for homecare services

    + Respond promptly to all phone calls, emails, and customer inquiries related to home care enrollment.

    + Log and route all incoming calls, documenting relevant information for reference and follow-up using Salesforce

    + Providing accurate and helpful information and serve as a point of contact for members.

    + Gather patient documentation and obtain any insurance information from the patient to get them cleared for services

    + Investigate type and level of insurance coverage to assess patient eligibility for home care services and educate members on available programs and the program enrollment process

    + Communicate with medical professionals and insurance company personnel to expedite patient eligibility into our program (remove)

    + Analyze weekly status update reports from all agency contracts and complete all necessary paperwork

    + Complete data entry to ensure accuracy of reporting and outcomes and schedule assessments and reassessments of applicants and enrollees using Salesforce

    + Upkeep timely communication with the Intake Manager, team members, and the marketing team.

    + Maintain constant exchange of information with various departments to schedule assessments, and gather required clinical and demographic data from patient and sources

    + Maintain effective relations with all agency contacts

    + Create, obtain, and track all communication notes using Salesforce

    + KPI and Performance evaluation will depend on the number of referrals converted to prospects

    Job Requirements:

    + High School diploma from an accredited school/ GED equivalent

    + Bilingual in Spanish is required

    + Experience working with Epaces & Salesforce is a plus

    + Has a sense of urgency in getting the job done

    + Personable and professional

    + Knowledgeable of CDPAP/MLTC programs

    + Experienced with the Intake process

    + Knowledge of the HHA eXchange is a plus

    + Able to work in a team-building environment
