Crouse’s Pharmacy team is hiring a Staff Pharmacist .

Pay Range:

Available Schedule(s):

+ Per diem – scheduled as needed, 7:00am-3:30pm; 3:00pm-11:30pm; 11:00pm-7:30am

Staff Pharmacist Responsibilities:

+ Completely evaluates all medication orders against clinically appropriate, patient specific parameters. Resolves any identified issues utilizing hospital approved guidelines or with the appropriate provider. Documents all interventions.

+ Initiates substitution of a formulary medication for a non-formulary medication when clinically appropriate.

+ Appropriately documents all clinical activities.

+ Reviews all orders for IV contrast as provided by Radiology.

+ Efficiently utilizes both hospital based and pharmacy based computer systems.

+ Assures that all products are appropriately labeled prior to dispensing.

+ Participates in maintaining a working stock of medications & supplies.

+ Ensures proper storage of medications.

+ Ensures that a patient, nursing unit or department is charged for all medications dispensed/ materials utilized.

+ Dispenses a 24 hour supply of unit of use medications to individual patients for all new orders.

+ Checks unit dose carts filled by pharmacy assistants. Checks prepackaged unit dose medications prepared by pharmacy assistants.

Staff Pharmacist Requirements:

+ current New York State Pharmacy License

+ Written and oral communication skill and interpersonal skills to deal effectively with other health care professionals and hospital employees.
