Licensed Practical Nurse
Job Description
The Licensed Practical Nurse provides routine nursing care, documentation, and record review. The role involves overseeing medical information, verifying follow-up, documenting results, performing chart reviews, and making appointments.
Responsibilities
+ Conduct comprehensive chart reviews to ensure accuracy and completeness.
+ Ensure documentation is accurate, with necessary follow-up noted.
+ Assist in maintaining files related to the medical care of individuals, adhering to the outlined Table of Contents.
+ Carry out charting functions for all medical matters related to individuals, including recording medical concerns and pertinent information.
+ Complete routine tasks such as processing doctors' orders, arranging follow-up appointments, tests, and specialist visits.
+ Maintain tickler systems, face sheets, BM charts, and self-medication checklists.
+ Schedule appointments as needed to facilitate patient care.
Essential Skills
+ Licensed Practical Nurse (LPN) certification.
+ Proficiency in electronic health records management.
+ Experience in healthcare, patient care, and patient education.
+ NYS RN License.
+ Minimum of 3 years RN experience.
Additional Skills & Qualifications
+ Experience working with underserved communities.

Work Environment
The work environment offers a standard 35-hour workweek, Monday through Friday, from 8 AM to 5 PM. Pay and Benefits
The pay range for this position is $25.00 - $35.00/hr.
