    **Job Description** EGA Associates, LLC. is currently seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide services at our Community Based Outpatient Clinic. The clinic typically operates Monday - Friday 0800-1630 and is closed for weekends and federal holidays. The general duty of the Nurse Practitioner (NP) or Physician Assistant (PA) is to provide professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) Primary Care Provider (PCP). The NP or PA is an Advanced Practice Generalist who provides a wide range of health services, including urgent and emergent evaluation of episodic and chronic problems; and comprehensive, integrated primary health care which addresses a majority of the client's health care needs on an ongoing basis. The NP or PA is knowledgeable in the assessment, treatment and evaluation of a comprehensive range of health care problems; emergency measures; preventive health; prescribing medications and other treatments; ordering, conducting, interpreting laboratory, diagnostic studies and procedures; and patient and family education utilizing community, social, and health resources. The NP or PA maintains knowledge of current practice, acts as role model and mentor to others, acts as client advocate, and participates in professional activities at local and state level. These duties include:Responsible for the provision of covered services to enrolled and unassigned patients presenting for care.Providing health care commensurate to the PCP’s licensure and clinical privileges or scope of practice.Ensuring the patient’s care plan contains medical recommendations for clinically indicated care, offering clinically indicated health care services to patients assigned to the PACT, and providing or arranging for care to which patients consent.Providing leadership to the team including shared delegation of appropriate care and care processes to appropriate team members.Reviewing available clinical and performance data with the team, and focusing on continuous improvement of critical team processes.Ensuring the patient has same-day access for face-to-face, telehealth, and telephone care visits during regular clinic hours, and collaborating with PACT staff to develop personal health plans that incorporate care management and care coordination appropriate to the patient’s needs.Keeps patients informed of their plan of care, including lab and diagnostic test results as needed.Provides health educational materials, resources and education to patients and their families for informational purposes and in accordance with their need and learning capacity.Assists the Registered Nurse to ensure all patient information on electronic encounter documentation is complete and accurate.Complies with all federal, state, local, Joint Commission, Occupational Safety and Health Administration (OSHA), Veteran Affairs, and Subcontractor safety and operational regulations, directives and standards.Participates in the ongoing Performance Improvement Program between the VAMC and EGA Performance Excellence program including regularly scheduled peer reviews.Ensures that all required reports and clinical information is completed in an accurate, timely and complete fashion.Participates in staff development programs.Performs other clinical duties assigned within limits of privileges.Maintains confidentiality of all information and supporting patients' privacy, rights, and safety.Aids in management/coordination of the CBOC's day-to-day activity.Performs other work-related duties as assigned.Qualifications:Must be willing to be trained in and perform Women's Health exams.Must be able to pass a VA background check.Must hold BLS certification through the AHA or MTN.Have proof of annual TB clearance and current flu vaccination or declination.CRNPMust possess a master’s degree from a program accredited by the NLNAC or CCNE and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body as a Nurse Practitioner in either Adult Health or Family Practice.Authorization for prescriptive authority is required.Three years of clinical nursing experience is required.A minimum of one (1) year clinical experience as a CRNP is required (three (3) years preferred).Experience in outpatient care in a Family Medicine or Internal Medicine environment is preferred.CRNP shall have current, full, active, and unrestricted license, and registration as a graduate professional nurse in the state in which the Outpatient Site of Care (i.e., CBOC) is located.PAMust be a graduate of a training program which is accredited by the Accreditation Review Commission on Education of PA (ARC-PA) or one of its predecessor organizations. Additionally, PAs must have a Master’s degree in any discipline or a Bachelor’s degree in any discipline with one year of experience as a PA.PAs must have successfully passed the PA National Certifying Exam (PANCE). Authorization for prescriptive authority is required.PA shall have current, full, active, and unrestricted license, and registration in the state in which the Outpatient Site of Care (i.e., CBOC) is located. Working Conditions: Moderate physical activity.
