Title: MSW or LCSW--Medical Social Worker Per Diem Farmington

What You’ll Do:

+ Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.

+ Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.

+ Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.

+ Implement patient care plan activities based on treatment goals towards independent living for the patient/family.

+ Serve as resource for staff education & training, participate in case conferences as needed.

+ Per Diem LCSW to support Palliative Care, Hospice, and Home Health

What You’ll Bring:

+ Master’s degree from a school of social work accredited by the council on social work education.

+ Maine Board of Social Work Licensure: LCSW Required.

+ Current CPR card required.

+ One or more years of experience as a master’s level social worker in a health care setting.

+ A valid Drivers License and reliable car with insurance.
