    Grand Junction Regional Center

     

    Grand Junction Regional Center (GJRC) is one of three Regional Centers in Colorado that serve people with intellectual and developmental disabilities who have intensive needs.

     

    GJRC, which dates back to 1919, has both Intermediate Care Facilities/Intellectual Developmental Disabilities (ICF/IDD) homes and Home and Community Based Services Waiver (HCBS Waiver) homes throughout the community. The center serves a diverse population, from people who have high abilities but are a high risk to themselves or others to people who have significant medical challenges.

     

    Programs are individualized to meet varying needs. Services provided range from residential services and health services to mental health intervention. GJRC provides day program, vocational and specialized habilitation, speech, occupational therapy, therapeutic recreation, and nutritional services.

    Description of Job

    The Quality Assurance and Performance Improvement (QAPI) Director at the Grand Junction Regional Center (GJRC) leads agency-wide compliance and quality initiatives to ensure adherence to state, federal, and division regulations. This role is responsible for safeguarding the rights of individuals served, analyzing systemic issues, driving performance improvement, and overseeing incident reporting, HIPAA compliance, policy development, staff training, and supervision. The QAPI Director plays a critical leadership role in fostering a culture of safety, accountability, and continuous improvement across the organization.

    Key Responsibilities:

    + Lead Incident Reporting Systems:Oversee critical incident reporting, analysis, and regulatory notifications, ensuring compliance with CDHS, CDPHE, and other oversight bodies.

    + Drive Quality Assurance and Improvement:Develop and manage the agency’s QAPI plan, conduct root cause analyses, and lead performance improvement projects (PIPs) to enhance service outcomes.

    + Monitor Compliance:Review and audit policies, procedures, and corrective actions for effectiveness and adherence to relevant laws and standards.

    + Develop and Deliver Training:Create and facilitate competency-based training for staff and individuals served, focused on rights, abuse prevention, HIPAA, and agency processes.

    + Act as HIPAA/Privacy Officer:Manage all aspects of HIPAA compliance, including investigations, reporting, staff education, and policy development.

    + Supervise Staff:Direct and evaluate a small team (3 FTEs), including hiring, performance planning, and implementing disciplinary action when necessary.

    + Serve as Duty Officer:Provide after-hours consultation and administrative decision-making during crises, ensuring safety and appropriate interventions.

    + Conduct Data Analysis:Identify trends across safety incidents, investigations, and program performance to inform strategy and improve care.

    + Maintain Regulatory Readiness:Serve as a liaison with oversight agencies, prepare the organization for audits, and manage responses to citations or findings.

    + Perform Other Duties as Needed:Assist with transportation, participate in system-wide meetings, and step into emergent needs across the organization.

     

    Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

    Experience Only:

    Eight (8) years of relevant experience in an occupation related to the work assigned to this position - such as experience in performing investigations, managing projects, analyzing data, judgment/decision making, monitoring/assessing performance, report writing, auditing, and writing plans of correction.

    OR

    Education and Experience:

    A combination of related education and/or relevant experience in an occupation related to the work assigned, equal to eight (8) years. Examples of related education include degrees in fields such as human behavior, human development, cultural studies, quality management, healthcare administration, health sciences, forensic science, criminal justice, business administration, or public health.

    Special Qualifications:

    + Must have a high school diploma and be at least 18 years old.

    Preferred Qualifications:

    + Experience working in service delivery systems for individuals with intellectual and developmental disabilities (IDD), including a minimum of five (5) years ensuring quality services.

    + Knowledge of Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) standards, interpretive guidelines, and survey processes.

    + Familiarity with Home and Community-Based Services (HCBS) waiver regulations and other acute or long-term care settings.

    + Demonstrated experience supervising or leading employees in a healthcare or human services environment.

    + Experience developing and implementing quality assurance or quality improvement plans.

    + Knowledge of applicable federal and state rules, laws, and regulations related to ICF/IID and HCBS services.

    + Proficiency in root cause analysis, data collection, and performance trend analysis.

    + Ability to develop written procedures, policies, and operational guidelines to ensure compliance.

    + Skilled in written and verbal communication, with experience presenting to varied audiences.

    + Proficient in computer systems and software, including Google Workspace and UKG.

    + Ability to manage time effectively, prioritize tasks, and make independent decisions.

    + Ability to facilitate change and lead workgroups or committees.

    + Flexibility in work hours to meet operational and emergency needs.

    Conditions of Employment:

    + Full Background - CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults –Mental Health Institutes, Regional Centers, Veterans Community Living Centers)

    + Pre-employment Physical/Tests – Exert up to 20 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects

    + Travel is required to the various places in which services and supports are provided, regulatory offices, and system-wide meetings. May need to assist staff and /or transport persons to meetings/ appointments.

    + Must possess a valid, unrestricted U.S. Driver's License

    + Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
