    Job Summary:

    The Patient Navigator – Annual Wellness Visit (AWV) Coordinator plays a key role in supporting preventive care efforts by identifying, engaging, and scheduling eligible patients for their Annual Wellness Visits within the primary care setting. This individual serves as a liaison between the care team and the patient, ensuring patients are aware of their eligibility, understand the value of AWVs, and are scheduled in a timely and efficient manner.

    Key Responsibilities:

    + Outreach & Engagement:

    + Identify eligible Medicare/ Medicare Advantage patients using EHR and practice/ACO reporting tools.

    + Proactively reach out to patients via phone, or patient portal to discuss and schedule Annual Wellness Visits.

    + Scheduling & Coordination:

    + Coordinate and schedule AWVs with appropriate primary care providers.

    + Ensure visit types are accurately categorized.

    + Send any necessary paperwork, including but not limited to, the HRA paperwork ahead of time.

    + Data Tracking, Recalls & Reporting:

    + Maintain accurate documentation of outreach and scheduled visits in the EHR.

    + Input patient recalls and follow up on them to ensure timely scheduling and continuity of care. Follow recall workflow and protocols.

    + Track metrics such as outreach attempts, scheduled appointments, completed visits, and patient declines.

    + Medicare Voluntary Alignment (Straight Medicare Only):

    + Educate patients on Medicare Voluntary Alignment and its purpose in selecting a primary care provider (PCP) for attribution.

    + Assist patients through the Voluntary Alignment process using CMS-approved tools (e.g., MyMedicare.gov).

    + Answer basic questions about alignment and help resolve common issues related to PCP selection or attribution errors.

    + Document alignment assistance activities accurately in the patient record and escalate complex issues to the appropriate staff or leadership.

    + Team Collaboration:

    + Work closely with site leads, front desk staff, clinical team members and quality engagement specialist to streamline patient experience.

    + Participate in site meetings as needed to stay aligned on outreach efforts and goals.

    Qualifications:

    + Education: High School Diploma or GED required; Associate’s Degree or higher in health administration, public health, or related field preferred.

    + Experience:

    + 1–2 years of experience in a healthcare or customer service setting required.

    + Prior experience with patient scheduling, care coordination, or Medicare services preferred.

    + Skills:

    + Strong communication and interpersonal skills.

    + Proficiency in EHR systems

    + Proficiency in Microsoft products (i.e., Excel, Word, etc.).

    + Detail-oriented with the ability to multitask.

    + Comfortable discussing preventive care with patients.
