Engagement Guide - Assisted Living Memory Care Residence (ALR) Rochester, NY

RPH is seeking an Engagement Guide to join our team at our assisted living communities located in Fairport and Rochester, NY. The Engagement Guide will deliver and oversee the delivery of a diverse array of daily therapeutic engagement activities that address the six areas of health – physical, mental, emotional, social, environmental, and spiritual that meet the needs, interests and well-being of elders living in a memory care setting. The Engagement Guide will support other staff in implementing scheduled activities and engaging with the elders. The Engagement Guide will help design and implement recreational and social opportunities ranging from small group programs to large special events and provide one-on-one engagement with elders as needed.

Essential Job Functions:
Elder Engagement
+ Organize and implement activities that meet the needs, interests, and history of elders as presented in the community calendar. Secure supplies for programming, maintain inventory and ensure storage is adequate, accessible, and maintained.
+ Assess elder’s individual interests to successfully implement recreation programs that meet their unique needs. Interview elders and/or family members to understand who an elder is, their history and activities they enjoy today and participated in in the past.
+ Develop engagement plans for each resident and review plans twice a year to adjust as needed over time. Maintain resident records of participation in engagement activities. Document resident notes as appropriate to keep team members and family apprised of the resident's experience. Engage with every elder.
+ Participate in community planning to identify programming related to the interests of elders, families, and staff with the goal of providing services to address the needs of the elders and their families.
+ Implement procedures for welcoming new residents to RPH, celebrating birthdays and other milestone events and marking elder departures/memorial services.*
+ Co-host resident council and family council with the administrator.
+ Involve interdisciplinary staff in the delivery of daily activities programs. Ensure Shahbazim and care partners have the supplies needed and sufficient instructions to implement programming. Guide Shahbazim and caregivers to deliver programming to elders.
+ Provide transportation for outings when necessary.
+ Oversee volunteers at programs and events to ensure compliance of policies and procedures.
+ Perform other duties as assigned.
*This may require attending evening and weekend events.
Community Engagement
+ Implement opportunities for elder and family engagement and multi-generational programming.
+ Support Human Resources team to execute special events that engage elders and staff. *
+ Represent RPH at senior care and other networking events in our community to promote our services. *
Requirements
+ A bachelor’s degree in a related field with a minimum of 1 year of experience in a social or recreation program within the last five years or similar.
+ Experience in an elder-care setting. Empathy to work with elders with memory impairment.
+ Possess a professional demeanor, and be an excellent communicator, quick learner, empathetic, compassionate, and savvy with a desire to achieve goals and make a difference in the lives of others.
+ Ability to work independently and collaboratively as part of a team.
+ Time management skills, good judgment, and a sense of humor.
+ Ability to be proactive, take initiative, resolve problems, follow through and manage multiple priorities and work well under pressure.
+ Must be able to assess and relate information concerning a resident's condition appropriately with the a bility to maintain confidential information.
+ Knowledge of Microsoft Office
+ Valid New York State Driver License with safe driving record – validated by RPH’s insurance company.
+ Successful NYS Department of Health Criminal History Background Check
+ Ability to work flexible hours to meet deadlines, including evenings and weekends. You may be required to attend evening and weekend events.
Physical Requirements:
+ Must possess sight/hearing senses that will enable these senses to function adequately so that the requirements of the position can be fully met.
+ Must be able to lift at least 50 lbs. Lift and relocate chairs, etc., and can safely assist elders in moving from one position to another.
+ Proficiency in English to communicate with elders, follow resident care plans and make reports.
