    Position Purpose:** ** ** ** 

     

    The PCA (uncertified, certified, unlicensed, or licensed) works in a collaborative environment at **Nazareth Hospital** to provide safe, quality health care services & / or assistance to patients under the supervision & direction of a registered nurse or other designated health care professional in accordance with level of experience, education, policies & procedures.

     

    3 Holy Family is a telemetry unit which specializes in Heart Failure. We do have heart failure certification.

    What You Will Do:** ** ** ** 

    + Employment Type/Shift: **Ft Nights**

    + Entry level position that provides training to function in an assistive role to the licensed nurse or other colleagues by performing patient care Activities of Daily Living (ADL) as delegated by the nurse.

    + Participates in continuous surveillance, rounding or monitoring for safety.

    + Offer compassion & patience as an essential member of the healthcare team (e.g., answering call lights, patient encouragement / greeting, stock / retrieve supplies or equipment, room turnover, mobilize patients, repositioning, transporting, discharge coordination).

     

    Conduct bedside report and LEAF repositioning for outgoing and oncoming PCA Round on each patient, introduce self to the patient and update white board; address immediate needs; Pass water pitches within 2 hours

     

    : Perform Vital Signs/ Accu Checks as needed/ Empty Foley

     

    Conduct Hourly Rounding/ Respond to Call Lights

     

    Reposition Patients at least every two hours or as instructed on LEAF monitor

     

    Refill supplies and gather equipment

     

    Assure cleanliness and organization of rooms and unit

     

    Perform Patient Hygiene/ Infection Control

     

    Admissions: check room for readiness, greet and orient patient, offer drink/snack if not NPO

     

    Discharges: remove laundry and used equipment

     

    Coordinate time off unit, breaks and your meals

     

    Perform vital signs for the next shift if there is only 1 PCA scheduled.

     

    Remain on the unit until you are permitted to clock out (23 minutes after the hour)

    Minimum Qualifications:

    + Currently enrolled in high school, completing a GED, or obtained a high school diploma or GED.
