**Monte Nido and Monte Nido Clementine** are treatment programs exclusively for adults and adolescents seeking treatment for Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder, or Exercise Addiction. Monte Nido’s treatment approach focuses on the restoration of physiological and nutritional balance, implementation of healthy eating and exercise routines, elimination of harmful behaviors, and development of motivation and treatment engagement. Our objective is to help each client achieve a clear understanding of their eating and/or exercise disorder and its effect on their life, as well as an individualized appreciation of what is necessary for their personal recovery.
We are seeking a Hybrid Social Worker to join our team.
The Social Worker will play a key role in providing care coordination and case management across **all Monte Nido Oregon sites** , serving both adults and adolescents and their families in our Intensive Outpatient (IOP), Partial Hospitalization Program (PHP), and Residential Treatment Center (RTC) programs. This role requires a dedicated professional to support the clinical team in ensuring smooth transitions between care levels and assisting clients and their families in navigating and accessing essential community resources.
Schedule: Monday-Friday 8:30 am-5 pm (Hybrid position)

Responsibilities Include:
+ **Care Coordination & Case Management:** Partner with the clinical team to offer case management and care coordination for clients and families, ensuring seamless access to community resources and services.
+ **Community & Family Support:** Assist in connecting clients and families with relevant community-based programs, educational resources, and supports from insurance or government programs, including but not limited to flex funds, transportation to care, and SNAP benefits.
+ **Referral Management:** Send referrals to community programs and resources, as well as help facilitate collaboration with outpatient providers and wraparound teams.
+ **Transitions Between Care Levels:** Help facilitate smooth transitions between various levels of care, such as from PRTS to community-based services, ensuring continuity of care.
+ **Prepare Clients & Families for Transition:** Provide necessary information and emotional support to both the client and family to prepare for these transitions.
+ **Ongoing Resource Connection:** Ensure that clients continue to receive the care they need after transitioning by connecting them with appropriate resources and services in their new care setting, following up post discharge from a Monte Nido Program.
+ Perform other duties as assigned.
Qualifications:
+ Master’s degree in Social Work or related field
+ LCSW preferred
+ Experience with CareOregon/Medicaid is a must
+ Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings.
+ Strong communication skills and the ability to work collaboratively with a multidisciplinary team.
+ Ability to build relationships with clients, families, and community providers to enhance client care and ensure successful transitions.
+ A strong understanding of available community resources, government programs, and insurance processes.
