    Proposed Schedule: 40 hours per week. Monday-Friday 8am-4:30pm

    Summary:

    The Care Management Program Coordinator will be responsible for the day-to-day administration (monitoring, reporting, quality, compliance, and communication) of the Care Management programs. This includes but is not limited to Health Home Adults, Health Home Serving Children, Health Home Plus, Medicare Chronic Care Management (CCM) and Patient-Centered Medical Home (PCMH) standards. In addition, the Care Management Program Coordinator will develop, monitor, and maintain care management reports and data used to track progress, quality, and efficiency alongside the management team. This role will also serve as a super-user for all Care Management documentation platforms and ensure training tools and guides are up to date.

    Essential Duties and Responsibilities:

    + Oversee the day-to-day functions of the Health Home and CCM programs, as well as ensure PCMH standards for Care Management are met. Responsibilities include maintaining an up-to-date knowledge of policies and procedures, maintaining accurate billing reports, assisting with quality and staff compliance, develop/update training guides, and communicating with the lead Health Home as needed

    + Establish and maintain strong relationships with Population Health and embedded Care Managers throughout the network to support the success of the Care Management programs and initiatives

    + Work with the Care Management Program Leader to implement policy and procedural changes that ensure effective program delivery and compliance

    + In collaboration with the Care Management Program Leader, provide training and ongoing professional development opportunities for staff related to care management programs and initiatives

    + Collaborate with healthcare providers, community agencies, and other organizations to ensure the needs of the care management patient population are met through referrals and transfers

    + Assist in the development and implementation of new initiatives, implement and maintain tracking, quality assurance, and evaluation systems, provide reports as requested

    + Serve as a member of various regional Health Home related workgroups and committees, attending meetings and educational/staff development events as appropriate

    + Provide outreach to patients for enrollment into care management programs or efforts related to our value-based programs for children and adult populations

    + Coordinate upward referrals to care management using a variety of data platforms such as: Relevant, Pearl, and other payer portals

    + Establish and maintain ‘super-user’ status for all documentation resources

    + Maintain knowledge of Care Management concepts, NCQA, PCMH, Health Home, as well as state and federal regulation/requirements

    + Assist with the management of the Health Commerce System applications as they pertain to Health Home: UAS, MAPP, IRAMS

    + Other duties as assigned

    Qualifications:

    The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:

    + Bachelor’s Degree required

    + Knowledge of NCQA, PCMH, Medicare CCM, and NYS Health Home required

    + Prior care management experience preferred

    + Strong verbal and written communication skills

    + Proficient computer competencies, including Microsoft applications

    + Well organized and effectively manage multiple cases and projects

    + Self-directed, detail-oriented and motivated

    + Able to work independently as well as collaborate and communicate effectively with colleagues, supervisors, service delivery partners, other health care professionals and co-workers to build and maintain effective professional relationships

    + Must have a valid driver license and be able to travel throughout the Network

     

    The pay rate for this position is $27.00/hour.
