Job Title:Reentry Case Manager
Employment Type:Full-Time, Exempt
Schedule:Monday – Friday, 8:00 AM – 4:00 PM
Job Summary:
FLACRA is seeking a dedicated and compassionate Reentry Case Manager to support individuals transitioning from incarceration at Ontario County Jail back into the community. This role is vital in connecting individuals to housing, treatment, employment, education, and care management services. The Reentry Case Manager will also ensure that jail contract obligations are fulfilled, services are effectively coordinated, and progress is tracked and reported.
Required Qualifications:
+ Associate’s Degree in Chemical Dependency, Criminal Justice, Psychology, Counseling, or a closely related field
+ Valid New York State Driver’s License
Preferred Qualifications:
+ Qualified Health Professional (QHP) status
+ CASAC certification or Bachelor’s Degree in Health or Human Services
+ Familiarity with jail-based services and community reentry programming
Skills and Experience:
+ Excellent verbal and written communication skills
+ Demonstrated ability to work with diverse populations and service providers
+ Strong knowledge of local behavioral health systems and community resources
+ Ability to multi-task, manage time effectively, and remain calm in crisis
+ Proficiency in documentation and use of electronic health records (EHR)
+ Strong interpersonal skills and ability to work independently and as part of a team
Job Requirements:
Key Responsibilities:
+ Facilitate access to community-based services before and after jail release, including behavioral health, primary care, and social supports
+ Provide screenings, assessments, and referrals; monitor engagement and follow up
+ Coordinate connections to care management, housing, employment, and educational/vocational training
+ Support benefit enrollment and recertification (e.g., Medicaid, DSS services)
+ Assist with addressing child support and family-related needs
+ Include families in reentry planning when safe and appropriate
+ Maintain understanding of jail contracts and ensure protocols are followed
+ Collect and report data accurately and on time; assist with best practice development
+ Provide therapeutic services as needed (individual, family, group counseling)
+ Attend staff meetings, trainings, and represent FLACRA in the community
+ Respond appropriately to crises to ensure client and staff safety
+ Complete all documentation in compliance with OASAS and agency standards
+ Perform additional duties as assigned
