Job Summary

Geisinger is seeking an experienced Nurse Practitioner or Physician Assistant for a per diem position in various skilled nursing facilities as needed.

Job Duties

This position requires an experienced advanced practitioner, prior experience working in a skilled nursing facility (SNF) is helpful but not required. The SNF advanced practitioner (AP) provides direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Overview:

+ Hours: Monday – Friday 8 a – 4:30p

Responsibilities include:

+ Communicate with SNF staff and review patient reports

+ Treat acute needs, admissions, discharges, etc.

+ Manage complex and non-complex patients

+ Average 12 patients a day

+ Length of time and number of patients as per need

+ Lakewood Rehabilitation & Healthcare Center, Nanticoke

+ Embassy of Wyoming Valley, Wilkes Barre,

+ Embassy of Scranton, Scranton

+ Oak Ridge Rehabilitation and Healthcare Center, Scranton

Position Details

Family Nurse Practitioner, Adult – Geriatric Nurse Practitioner or Physician Assistant with advanced practice experience in a skilled nursing facility or long-term care facility is preferred.

Master's Degree-Nurse Practitioner (Required)

Experience

Certification(s) and License(s)

Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Certified Nurse Practitioner - American Academy of Nurse Practitioners; Basic Life Support Certification - Default Issuing Body
