    Trinity Health: Mid-Atlantic and Holy Cross Health Maryland

     

    To leverage skill and scale and expand services across a broader geography, in May 2024, Trinity Health combined the responsibility for its Mid-Atlantic Region (Pennsylvania and Delaware) and its Holy Cross Health in Maryland Region under one Regional Health Ministry (RHM) leadership team. Combined, these two regions have six hospitals, as well as, associated clinics, medical offices, specialized facilities, affiliated institutions, and foundations, representing approximately 10,400 colleagues across three states, with approximately $1.7B in total revenue. These facilities and colleagues serve together in the spirit of the Gospel as a compassionate and transforming healing presence within their communities.

    POSITION PURPOSE

    The Regional Vice President of Community Health & Well-Being (CHWB) advances Trinity Health Mid-Atlantic’s vision to be a national leader in Community Impact by removing barriers to care, reducing health disparities, and improving outcomes for people experiencing poverty and other vulnerabilities. This role leads the integration of clinical and social care with dignity and cultural awareness, implementing scalable, evidence-based approaches to prevention, chronic disease management, and social care integration.

     

    The VP drives strategy, operations, and partnerships that align with Community Health Needs Assessments (CHNAs) and Implementation Strategies—investing in community-identified priorities, advancing policy and systems change, and ensuring compliant and impactful Community Benefit reporting. Through collaboration, advocacy, and innovation, this leader ensures Trinity Health delivers measurable, lasting improvements in community health and well-being.

    CRITICAL FUNCTIONS

    + Design and operationalize Social Care service delivery models for Trinity Health patients that effectively integrate with clinical care and leverage partnerships with community-based services that mitigate the social needs of vulnerable populations to improve health outcomes.

    + Lead the development of policies, systems and environmental change strategies that are community-based and community partnered initiatives/innovations that center on promoting TogetherStrong for the Common Good.

    + Facilitate the development of collaborative, community driven hospital triennial community health needs assessments and implementation plans that are designed to measurably improve outcomes.

    + Ensure robust, timely and accurate community impact reporting, inclusive of community benefit.

    STRATEGIC FUNCTIONS

    + Facilitate, strengthen and sustain internal partnerships (i.e. clinically integrated network, medical group, advocacy, mission, marketing/communications, finance, supply chain, etc.) to integrate and advance CHWB strategy with our Mission and Core Values, and other Ministry goals.

    + Identify and advance internal policy and practice changes to improve health and well-being for people experiencing poverty and other vulnerabilities.

    + Develop and sustain strategic and authentic community partnerships that advance clinical and social care integration and improves community conditions through policy, system and environmental change strategies.

    + Convene RHM leadership to advance anchor strategies (local hiring, local sourcing, and local investing) across the region.

    + Enhance evidence-based health promotion and disease prevention interventions.

    + Lead sustainability efforts through strategic fundraising, grant writing and billing for eligible services for CHWB programs and initiatives.

    ESSENTIAL FUNCTIONS

    + Knows, understands, incorporates and demonstrates the Trinity Health Mission, Vision and Core Values in behaviors, practices and decisions.

    + Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical and professional behavior.

    + Accountable executive for CHWB. Interviews, hires, orients, oversees training and development, schedules, assigns work, evaluates performance and, when necessary, disciplines, all direct reports. Ensures compliance with established Hospital and personnel policies when operating the Department.

    + Directs the development and implementation of a framework to connect clinical and social care across the region, including the development and sustainability of a community health worker infrastructure. This framework includes: The coordination and integration of community health workers to ensure collaborative and effective care for people who are dually enrolled in Medicare and Medicaid and other vulnerable populations we serve. Developing and strengthening partnerships to ensure a seamless system of social care support for the people we serve that includes multiple partners: ambulatory leaders, local and state government, academic institutions, payers, community-based organizations, and faith-based organizations and beyond.

    + Serves as accountable executive leading/facilitating the development and implementation of the triennial Community Health Needs Assessment (CHNA) and Implementation Strategy in partnership with community stakeholders, placing particular emphasis on centering health and racial equity throughout the entire process and focusing on populations and geographies experiences poverty and other vulnerabilities by advancing policy, system and environmental change strategies that improve community conditions and individual health and well-being.

    + Serves as the accountable executive for leading the development and implementation of community benefit policies, procedures, tools and training across Regional Health Ministry to ensure compliant, comprehensive and accurate reporting of s activities as required by IRS regulations and Trinity Health standards.

    + Identifies and establishes rapport with funding sources such as foundations, state and federal agencies and establishes reporting systems for each funded initiative in order to ensure that programmatic designs comply with all applicable rules and regulations. Actively seeks external funding to support the advancement of the CHWB strategy, including billing for eligible services.

    + Represents Regional Health Ministry at professional or community organization meetings and on committees at local, state and national level in order to assist in developing new programs and policies for improved delivery of health services for those who are experiencing poverty and other vulnerabilities.

    + Develops and cultivates partnerships with grassroots organizations and Black, Indigenous, People of Color (BIPOC) led/serving organizations to ensure services and priorities are culturally relevant and sustainable.

    + Identifies and promotes desired changes in the Regional Health Ministry’s culture and programs that promote diversity, improve cultural proficiency, promotes health and racial equity and decreases healthcare disparities.

    + Serves as a key member of the Regional Health Ministry leadership team, participating in strategy development, planning and decision making, leading selected Ministry-Wide initiatives and providing communication to the local RHM Board(s) at least quarterly to ensure alignment with strategy, support for CHWB activities and compliance with corporate and government requirements.

    + Serves as the leader and accountable executive for the Healthy Village at St. Francis.

    + Participate as the CHWB representative on the Regional Executive Leadership Team and Regional Integrated Clinical Leadership Team.

    MINIMUM QUALIFICATIONS

    + At least 10 years of experience in a leadership level/supervisory capacity with proven strategic planning, fiscal management and leadership capacity.

    + Master’s degree in Health Care Administration, Public Health, Community Health or related field required.

    + Experience developing and implementing innovative initiatives that improve population and community health.

    + Must be able to operate effectively in a collaborative, shared leadership environment.

    + Must possess a personal presence that is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health.

    + Strong working knowledge of health and racial equity principles, social influencers of health, uncompensated and undercompensated care, Community Benefit, community and coalition building, public policy development and advocacy, and population health management concepts.

    + Excellent communication (interpersonal, verbal and written) skills.

    + Excellent program design and evaluation skills.

    PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

    + Must be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Health Ministries (HMs).

    + Must possess good manual dexterity to operate a computer keyboard.

    + Must possess good hearing to be effective on extensive telephone conference calls and for in person communication.

    + Must be able to travel to various Regional Health Ministry sites and other locations as needed. Approximately 50% of the time.
