    Summary The Program Manager, Hematology/Oncology Pharmacy Program is a licensed pharmacist who is responsible for planning, directing, coordinating and overseeing the clinical and operational activities for the Pharmacy Hematology/Oncology Section. The Program Manager, Hematology/Oncology will also serve in the role of a Clinical Pharmacist Provider (CPP) for predominantly outpatient oncology pharmacy care activities including the Infusion Center. Responsibilities Program Management Develops, organizes, manages, and controls complex hematology/oncology pharmacy programs. Coordinates the clinical activities for the Hematology/Oncology Clinical Pharmacy Section. Maintains adequate drug stock, inspects drug storage areas, supervises prepacking and the operation of automated dispensing equipment. Monitors and analyzes orders and distribution patterns to identify questionable circumstances or unusual trends (including waste, fraud, or abuse) and works with appropriate committees and services to address. Maintains effective and economical inventory management. Ensures that inventory levels of medications and medical supplies are managed so that patient care is not impaired due to insufficient stock or the unavailability of a therapeutic alternative. Ensures all other medication-related automation systems (Alaris pumps, etc.) are updated and functioning properly. Serves as a resource for staff on proper use. Clinical Functions at the highest level of clinical practice and works independently under the oncology area scope of practice. Provides medication reconciliation and appropriately renders care to the Oncology Clinic and Infusion Center patients including appropriate computer documentation and follow-up activities. Works towards achieving positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in Hematology/Oncology management in the outpatient clinics and/or the inpatient areas. Responsible for evaluating and communicating prescribed drug therapy on a daily basis to physicians and pharmacists for appropriateness, effectiveness, cost-benefit, adverse effects, allergies and disease states. Plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting in writing therapeutic plans utilizing the most effective, least toxic and most economical medication treatments. Orders and interprets laboratory and other diagnostic tests to help determine efficacy and toxicity of medication therapy. All patient interviews, assessment notes, and clinical activities, are completed appropriately and in a manner consistent with the current quality assurance policies and are documented in writing in the medical record. Dispensing/Operations All inpatient/outpatient orders are checked to include patient information, correct medication, dose, route, and frequency of administration. Utilizes VCM (Vista Chemotherapy Manager) software to process chemotherapy orders. Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutics and formulary. Documents interventions as necessary. Fills orders in a timely fashion, based on priority. Ensures drugs dispensed are completely and correctly labeled and packaged in full compliance with Education/Training: Responsible for developing and managing all pharmacy educational programs pertaining to the advancement of appropriate pharmacological oncology therapy which includes but is not limited to medical and pharmacy residency training programs, fellowships, clerkships, internships, management training programs, staff development programs, university affiliated programs, and interdisciplinary training programs. Provide formal and informal education and in-services to pharmacy, medical, nursing, and other staff as requested. Supervise and provide training for pharmacy residents, pharmacy students, medical residents, medical students, and other allied health care professionals participating in the treatment patients within the Hematology/Oncology service. Serves as a drug information resource to the Hematology/Oncology staff, by providing up-to-date information to prescribers, other health care professionals, patients and caregivers. Trains staff pharmacists to function in areas of direct patient care and clinical services. Basic Requirement United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: 1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. 2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C 7402(d), and 7407(d). Grade Determinations: In addition to the basic requirements, the following criteria must be met when determining the grade of candidates. GS-13 Facility Program Manager EXPERIENCE. In addition to GS-12 requirements, must have 1 year of experience equivalent to next lower grade level (GS-12) AND KNOWLEDGE, SKILLS, and ABILITIES (KSAs). Must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. NOTE: GS-12 Requirements Education/Experience. 1 year of equivalent experience to next lower grade level (GS-11) OR OR completion of an ACPE- accredited Pharm. D. program. AND demonstrated the following KSAs: Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
