Position Overview
The LEAP Assessment Coordinator (AC) is a part-time role (16 hours per week) dedicated to conducting clinical assessments for patients in the RISE clinic at CHA, a specialized program for individuals experiencing first-episode psychosis. The AC plays a key role in overseeing the assessment process for all patients entering the RISE program and conducting follow-up assessments every six months.
Key Responsibilities
Clinical Responsibilities
+ Conduct structured clinical interviews and administer self-report questionnaires to patients.
+ Schedule and facilitate virtual or in-person assessment sessions (1-2 hours per patient).
+ Attend clinical team meetings.
+ Coordinate assessment feedback with team clinicians.
Administrative Responsibilities
+ Manage and maintain the assessment database.
+ Track patient assessment schedules and ensure timely completion.
+ Monitor and follow up on RISE patients due for assessments.
+ Participate in weekly meetings with Assessment Directors to review progress.
+ Provide monthly updates on assessment engagement.
+ Attend weekly diagnostic meetings.
Qualifications
+ Minimum Bachelor’s degree in a social science or related field with an interest in healthcare.
+ Prior experience or strong interest in working with young individuals (ages 15-30) with serious mental illness.
+ This position is ideal for individuals seeking hands-on experience in clinical assessment within a multidisciplinary mental health setting.
+ Familiarity with RedCap, Excel and other database management programs strongly preferred
