Join our team as a Program Coordinator leading Assertive Community Treatment (ACT) Teams and Health Home Services. In this key leadership role, you’ll oversee multidisciplinary teams providing integrated care to individuals with serious mental illness and complex medical needs. We’re looking for an experienced LCSW with strong behavioral health leadership skills and a passion for recovery-oriented, trauma-informed care.

• Location | Glen Cove based but travel throughout Nassau County
• Full-Time | Exempt
• Monday–Friday | On-call rotation required
• Salary | $85-90,000 annually
About the Role
We’re seeking a mission-driven Program Coordinator to lead our Assertive Community Treatment (ACT) Teams and Health Home Services. This leadership role is responsible for the clinical and operational management of multidisciplinary teams serving individuals with complex behavioral health and medical needs.
You’ll oversee ACT program delivery in alignment with OMH standards and guide the performance of our Health Home Care Management program in partnership with Northwell. What You'll Do
+ Oversee daily operations of ACT teams and Health Home services
+ Supervise and support psychiatrists, nurses, social workers, peers, and care managers
+ Ensure compliance with OMH, DOH, CMS, and Health Home regulations
+ Implement quality improvement and staff development initiatives
+ Foster a culture of collaboration, accountability, and trauma-informed care
+ Represent programs in audits, site visits, and external stakeholder meetings
+ Participate in on-call rotation
What You Need
• Master’s in Social Work (MSW)
• NYS Licensed Clinical Social Worker (LCSW)
• 3+ years of leadership experience in behavioral health
• Experience with ACT programs strongly preferred
• Valid NYS driver’s license
Key Skills

+ Proficient in Microsoft Office & EHR systems
+ Deep understanding of OMH/Health Home regulations
+ Ability to work in fast-paced, multidisciplinary settings
