    Job Overview:

    Responsible for the functioning of Oncology Breast consultation program. Role includes patient/family education, assistance with clinical examinations, coordination of consultation team meetings, and cancer support groups and genetics program. Responsible for active participation in the consultation program ongoing management of its activities. Responsible to coordinate patient records, reports and diagnostic information. Works to establish and maintain relationships with community programs, physicians and other agencies. Participates in the accrual process for research protocols.

    Required:

    + Current New Jersey RN license

    + Bachelors Degree required

    + BCLS required to start

    + Certified by the American Credentialing Center in an appropriate area of specialty or by the Oncology Nursing Society as an OCN required

    Preferred:

    + Masters degree preferred.

    Schedule:

    + 40 hours a week - Days

    + Weekends and holidays may be required

    Job Specific Requirements

     

    Completes the initial assessment in it s entirely as applicable to unit within specified time frames.

     

    Identifies patient problems, makes appropriate referrals, sets achievable goals, and develops individualized plan of care.

     

    Evaluates and reassess patient s progress toward goal attainment along with members of the interdisciplinary team.

     

    Provides direct and indirect patient care activities safely and competently according to identified standards / polices / procedures and within scope of role.

     

    Facilitates timely completion and accurate transcription of orders and diagnostic testing and co-signs according to policy and procedure.

     

    Documentation is patient specific and problem focused; chart entries are legible, complete, and accurate.

     

    Knowledge regarding chemotherapy drugs; i.e., acute and chronic side effects and its methods of prevention, principle therapeutic use, and risks of extravasations.

     

    Prepares patients for procedures; provides adequate explanations and time for questions.

     

    Educates patients and families to meet identified learning needs.

     

    Facilities timely flow of patients through unit and makes every effort to accommodate patients in a timely manner.

     

    Interprets and utilizes clinical data in the plan of care.

     

    Assesses patient for the potential risk factors.

     

    Responds effectively in an emergency situation so that the patient s immediate needs are addressed.

     

    Assesses learning needs, readiness to learn and actual or potential barriers to learning.

     

    Communicates and documents change in patient s condition to all appropriate individuals.

     

    Recognized as an advocate of teamwork.

     

    Obtains upon employment and maintains unit/department competencies and standards.

     

    Delegates work to the care team.

     

    Collaborates with other interdisciplinary team members to affect positive patient outcomes.

     

    Effectively interacts with physicians; provides complete information when consulting orders or changes in condition.

     

    Documents timely in a clear and legible manner.

     

    Documents assessment/reassessment, plan, interventions and outcomes according to policy.

     

    Responds to and documents emergency interventions according to policy.

     

    Documents medication use and controlled substance per policy.

     

    Medication reconciliation.

     

    Recognizes and integrates patient special needs and individualizes care.

     

    Includes pain management and supportive care issues in the plan of care.

     

    Prioritizes and organizes work assignment and adjusts priorities based on changing situations.

     

    Consults and refers to other health care professionals.

     

    Plans delivery of care according to patient acuity and staff competencies.

     

    Integrates primary and secondary diagnosis into the educational plan.

     

    Individualizes educational plan based on age, cultural and spiritual values.

     

    Adheres to the infection control standards specific to patient care.

     

    Assumes accountability for managing the delivery of patient care.

     

    Refers ethical concerns to appropriate resources.

     

    Demonstrates skill in problem-solving and priority recognition.

     

    Adheres to documentation and policy as it relates to regulatory compliance, i.e., CMS, The Joint Commission, Dept. of Health, etc.

     

    Utilizes effective time management skills to complete work within a predetermined time frame.

     

    Acts as a patient advocate and upholds patient s rights.

     

    Promotes and maintains a safe, clean and orderly environment.

     

    Performs as a mentor when necessary.

     

    Communicates transfer of patient responsibility shift report guided by the plan of care or any other pertinent documents (i.e. SBAR walking shift report)

     

    Appropriately utilizes resources and effectively delegates to team members; follows up on timely completion of assigned duties.

     

    Demonstrates initiative and motivation in role, proactively addresses issues, identifies and resolves problems in a satisfactory manner.

     

    Responsive to patient needs; (example: answers call bell timely, keeps noise levels at a minimum, provides privacy and confidentiality).

     

    Is flexible and responsive toward needs of department, and assists in every way to meet patient care requirements.

     

    Is self-directed toward educational opportunities; keeps education record current.

     

    Regularly attends staff meetings and contributes positively to unit goals.

    Networks with existing cancer care services for the purposes of developing and maintaining.

     

    Navigates the patient through the hospital system to ensure continuity of care.

     

    Participates in tumor board conferences.

     

    Presents educational programs regarding services available through the consultation program and genetic program.

     

    Provides/facilitates educational opportunities for patients, staff and community.

     

    Develops and maintains applicable polices and procedures.

     

    Generates appropriate and required reports.

     

    Participates in Cancer Committee.

     

    Collaborates with research coordinator for accrual of patients.

     

    Identifies and addresses patient satisfactions issues; directs efforts toward improving patient satisfaction scores.

     

    Participates in on-going documentation review.

     

    Achieves or maintains professional certification specialty area.

     

    Actively assists in the orientation and preceptorship of new employees.

     

    Participates in Community based events.

     

    Participates in Educational opportunities.
