    Salary: $45 / Hour

     

    Are you a compassionate RN with a passion for primary care and care coordination? A well-respected rural healthcare organization in Orleans County, New York is seeking a RN Triage Care Manager to join a dedicated team and make a real impact on the lives of patients in the community.

     

    In this role, you'll work closely with a diverse patient population, providing triage, care coordination, and support to those most at risk. You'll be an integral part of a supportive environment where your clinical expertise and leadership truly matter.

     

    + Salary: 75k-90k

    + Schedule: Monday-Friday; 8a-4p; NO weekends or holidays

    Minimum Requirements:

    + Current New York State Registered Nurse license

    + Background in any of the following: triage, case management, disease management, home health care, nursing, hospital nursing or intensive outpatient education and/or self-management support

    + Current CPR

    + TB Questionnaire, PPD or chest x-ray if applicable

    + Current Health certificate (per contract or state regulation)

    + Must meet all federal, state and local requirements

    + Must be at least 18 years of age
