General Description
The purpose of this position is to fulfill the duties for the Employer Resource Connection program as outlined in the Agreement set forth between Oregon Home Care Commission and Northwest Senior and Disability Services. The Employer Resource Consultant (ERC) will help consumers of in-home services become effective employers of in-home care workers, thereby helping to ensure quality home care services for seniors and people with disabilities. ERC will help guide consumers to connect to service agencies to obtain services needed.
Essential Functions:
+ Partner with OHCC to promote the ERC Program
+ Conduct outreach programs
+ Conduct training workshops
+ Partner with Agency employees on Referrals for processing
+ Provide comprehensive support for the consumer
+ Determines the need and appropriateness of ERC participation
+ Supports participants’ success with program and in-home services
+ Protects clients and reduces Agency risk
+ Provides excellent customer service in a professional manner
+ Submit monthly billing report to Agency for submission to OHCC
1. Partner with OHCC to promote the ERC program
+ ERC will participate in OHCC required meetings, including technical assistance meetings.
+ Thoroughly understanding the program objectives, values and benefits
+ Developing and conducting outreach to the community, possible participants and others that can provide referrals to program including CM mini meetings and workshops, etc. .
+ Knowledge of in-home services, including the role of the Home Care Worker, the Home Care Commission and the consumer employer
+ Understanding of the role of individual’s preferences in care success
+ Knowledge of the ERC program, including purpose, objectives and eligibility requirements; preparation of materials for and conducting public speaking.
+ Utilizes social media to distribute OHCC approved outreach
2. Conduct outreach programs
+ ERC will need to conduct at least five outreach activities provided in one or more of the Agency’s assigned areas.
+ Provide quarterly written reports to OHCC highlighting outreach activities, best practices and success stories.
3. Conduct training workshops
+ ERC will be responsible for holding at least one workshop annually in each county within the Agency’s assigned areas.
+ ERC will create topic workshop presentations based on content from OHCC materials.
4. Partner with Agency employees on referrals for processing
+ ERC will process referrals timely.
+ Attempts to schedule an appointment with the consumer must be made within five business days
5. Provide comprehensive support for the consumer
+ ERC will complete a consumer checklist for each consumer to assist in determining the types of services that are requested.
+ Consumer checklists will need to sent out to the consumer/employer, the representative, the Case Manager and OHCC
+ Checklists will be sent to all parties within three business days of the initial meeting.
+ Collaborating with Consumer, Case Manager, authorized representative (when applicable) and others in developing a plan that honors the consumer’s choice of services, desired location, etc., while addressing identified barriers in managing a Home Care Worker
+ Providing services as agreed upon by consumer or representative
+ Providing follow-up and monitoring for consumers with complex service plans to prevent in-home service plan failure
+ Working with Case Manager in monitoring and evaluating changes in needs and services.
+ Available community resources and parameters for populations served
6. Determines the need and appropriateness of ERC participation
+ Conduct a an Initial Meeting to assess and determine their ability to manage a Home Care Worker (which may include gathering appropriate information from others involved, such as family, representative, Case Manager, Protective Services Worker) and identifying any barriers to effectively managing a Home Care Worker
+ Ability to learn software and programs related to assessment and plan development; working in a team setting to achieve positive outcomes.
7. Supports participants’ success with program and in-home services
+ Providing group and individual training on being an employer of a Home Care Worker through workshops
+ Defining the roles and relationship of employer and employee
+ Providing ongoing educational opportunities and resources for participants.
8. Protects clients and reduces Agency risk
+ Following policies, regulations and requirements of program and Agency
+ Documenting on an accurate and timely basis as required by Federal, State, funding regulations, and Agency policy
+ Compiling accurate data and preparing reports on outreach activities, referrals, Initial Meetings, Consumer Checklists, Case Manager communications, etc.
+ Serving as a Mandatory Reporter of suspected abuse of vulnerable populations according as required by policy and regulation.
+ Maintaining and sharing information according to privacy regulations.
9. Provides excellent customer service in a professional manner

+ Exhibiting good work habits; following policy and procedure
+ Working independently, seeking and offering assistance when needed; exhibiting technology skills related to the work needing to be done, (word processing, spreadsheets, database, internet research, mail, IM, phones, copiers, assessment programs like CAPS, Oregon ACCESS, etc.)
+ Exhibiting a positive attitude towards clients, co-workers and others.

10. Submit monthly billing report to the Agency for submission to OHCC
+ Bill for all direct services and allowable administrative services or tasks and workshop and workgroup activities.
+ Billable expenses related to services
+ No-show appointments are billable
+ Bill for excess travel
+ Bill for in-person in-service day at request of OHCC
+ Bill for reimbursements for lodging, mileage and meals
General Skills Needed to Meet Agency Expectations
Must be able and willing to perform the following:
+ Support the agency mission and exemplify its core values—integrity, professionalism, service, and compassion
+ Serve as a mandatory reporter of suspected cases of neglect, exploitation, and abuse of vulnerable populations as required by policy and regulation, and make the appropriate referral to a responsible agency such as Child or Adult Protective Services Units
+ Interact and work effectively with others in a team to deliver services to consumers
+ Demonstrate good interpersonal communication skills through written, verbal, and nonverbal communication
+ Use sound organizational skills to meet deadlines in a timely, accurate, and efficient manner
+ Speak, read, write, and understand English and follow verbal and written instruction
+ Perform other work as assigned by the supervisor
+ experience working with seniors and people with disabilities and the issues experienced by these populations (social, physical, care, etc.)
+ ability to interview others, observe and assess situations accurately
+ ability to prepare effective plan and facilitate agreement between those involved
+ developing and conducting training
+ understanding of ADRC and ERC program
+ good interpersonal skills, interacting in respectful and culturally appropriate way
+ knowledge of rules around documentation and mandatory reporting
Required Experience:
Minimum Qualifications - Experience and Education
A qualified applicant will have a minimum of six (6) years of equivalent combination of education and/or experience which demonstrates the knowledge, skills and abilities required. The following qualifications meeting the minimum requirements will be considered:
+ Bachelor’s degree in social sciences or related field of study
+ Two years of experience working with social service programs in a similar capacity with similar populations.
(For Bilingual positions only)
• Successful completion of a language proficiency test.
While not required, experience working with seniors or adults with disabilities or vulnerable populations is a plus.
Work Environment and Physical Demands
Employer Resource Connection Consultants work both in an office environment and in the community. They:
+ Use a computer, telephone, and other office equipment
+ Need to tolerate and be able to work where the noise level is that of a typical office
+ Work on site at a social services entity and health care facility
+ May encounter frequent interruptions throughout the work day
+ Are regularly required to sit, talk, or hear
+ Use repetitive hand motions
+ Must be able to handle objects and sustain a sense of touch
+ Must be able to stand, walk, reach, and bend
+ Must be able to lift up to 20 pounds
+ Requires travel to all service areas of Agency
Contact with the public in homes, facilities, or office environments may risk exposure to people with contagious diseases or irrational/hostile behavior and contact with domestic animals.

To be successful, candidates must:
+ Secure and maintain a valid Oregon driver’s license or have an acceptable alternative means of transportation
