    Intensive Mental Health Provider (Part-Time)

     

    The Opportunity

     

    Family and Children’s Association (FCA) is seeking a passionate Part-Time Intensive Mental Health (IMH) Provider to join our Y-Fi Program, supporting Suffolk County families with children ages 5–21 who have or are at risk of serious emotional disturbances.

     

    The IMH Provider will connect families to community resources, ensure needed supports are in place to maintain the child in the home, and deliver skill-building services as needed.

    Responsibilities

    + Conduct and ensure compliance with all visit requirements for children, families, and service providers.

    + Provide skill-building services to clients, aligned with their individual service plans and needs.

    + Perform ongoing assessments and document the strengths, needs, and progress of both the child and family, evaluating the effectiveness of delivered services.

    + Coordinate and oversee Family Support and Youth Peer Advocacy Services, including active participation in monthly team meetings.

    + Connect clients with appropriate services and resources, facilitating necessary referrals and linkages.

    + Complete and submit all required documentation, including reports, progress notes, and service plans, in a timely manner and in accordance with agency and regulatory standards.

    + Maintain up-to-date and accurately filed client records in compliance with program and OMH guidelines.

    + Keep the Coordinator informed of case progress, promptly reporting any unusual situations, including potential child protection concerns or safety risks.

    + Perform additional tasks and responsibilities as assigned to support the efficient operation of the program.

    Qualifications

    + Bachelor’s degree in a Human Services-related field.

    + Minimum of two years of experience providing direct services to children with emotional impairments.

    + Proficiency in Microsoft Office Suite, including Word and Excel, with intermediate computer skills.

    + Valid and clean driver’s license required.
