Description:
**Asylum Hill Family Medicine a member of Trinity Health Of New England Medical Group** is looking for **LCSW Social Worker** to join our **Family** **M** **edicine** team.
The **Licensed** **Clinical Social Worker** will work collaboratively with patients, families and the medical team to ensure patients recover and achieve optimal well-being. The ideal candidate will display a drive to provide patient-centered experiences through scholarly scientific inquiry, compassion for the human spirit and mind, advocacy and education for the patient and families, and collaboration with all those who touch our patients.
You Will Be Responsible For:
+ Perform psychosocial assessments, counselling, crisis intervention, case management, patient support, education
+ Connects patients to resources within the community
+ Provides support to patients and families and is a patient advocate to assure appropriate cares is received.
+ Effectively communicates with patients, families and the integrated clinical team to ensure everyone understands the needs of the patient to ensure a safe care plan is created.
+ Collaborates with care team, patient’s physicians, therapists, nurses, case managers, integrative medicine, and spiritual care personnel to revise the care plan as necessary.
**Minimum Requirements** :
+ MSW degree from an accredited School of Social Work required.
+ CT State Social Work Licensure LMSW or LCSW preferred.
+ Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.
+ Knowledge of Medicare, Medicaid, and managed care regulations and trends
+ Experience with utilization management, home care, community resources, and rehabilitation beneficial.
+ **Bilingual (Spanish) preferred**
