    Description:

    POSITION PURPOSE

    Provide high-level service care coordination to psychiatric patients by supporting patients and others through adequate care and being able to assist with critical tasks while serving as a resource to physicians and other team members. Provide knowledge about recovery, cultural competency, person-centered treatment, and trauma-informed care approaches.

    ESSENTIAL FUNCTIONS

    Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

     

    Provides coordination of care to clients through scheduling appointments, rescheduling, follow-ups, and collaboration amongst physicians and clients.

     

    Completes prior authorizations to insurance companies and submits refill forms to the pharmacy as necessary.

     

    Collaborates with the House of Mercy's treatment team regarding client care needs.

     

    Assists with critical tasks related to medication management and oversight of residential medication room processes and procedures.

     

    Maintains regular contact with the manager and physicians to support a continuum of care.

     

    Provides support to clients and families regarding psychiatric service needs through face-to-face interactions, emails and/or phone.

     

    Participates in clinical staffing, case management meetings, and action plan sessions, as needed.

     

    Serves as a resource to other members of the treatment team in the area of medication management and psychiatric services.

     

    Documents all activity in a qualitative/timely manner through our EHR system.

     

    In partnership with the medical director/manager, ensures all CARF accreditation standards and State regulations are being met for psychiatric services.

     

    Maintains knowledge about recovery, cultural competency, person-centered treatment, and trauma-informed care.

     

    Coordinates ongoing patient flow through House of Mercy, ensuring efficient and effective continuity, accurately completing all required forms, greeting clients, verifying demographics and insurance information, and coordinating physician releases.

     

    Provides support for internal departments with record requests within our EHR system.

     

    Participates in House of Mercy’s Performance Quality Improvement Process.

     

    Assists with the development/completion of grant applications, funding appeals, and reports required by grantors, if and when appropriate.

     

    Gathers, maintains, and reports statistical data and completes reports related to ongoing research, grants, and purchase of service agreements, as requested.

     

    Participates in public relations and media efforts to promote House of Mercy; attends community and informational meetings to network with other providers; attends training on relevant topics to stay current on trends related to the job; meets licensing/regulatory requirements; and obtains information to better serve clientele.

     

    Assists the Leadership team in developing programs and/or curriculum, as directed.

     

    Proactively seeks opportunities to improve/enhance programming and makes recommendations to the Leadership team for improvements/enhancements.

     

    Other duties as needed and assigned by the manager.

     

    Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

    MINIMUM QUALIFICATIONS

    Bachelor’s degree in a human service-related field from an accredited four-year program, preferred.

     

    Clinic/physician office experience, preferred.

     

    Previous experience working with chemically dependent or “at risk” clientele, preferred.

     

    Ability to understand medical terminology and communicate effectively with healthcare professionals, preferred.

     

    Iowa Department of Health and Human Services Mandatory Reporter Child and Dependent Adult Abuse training within 30 days of hire.

     

    Must be comfortable operating in a collaborative, shared leadership environment.
