Job Description​

The Opportunity:

Saint Luke’s Advanced Care Pharmacy has a career opportunity for a highly qualified individual/pharmacist. This is a full-time position (1 FTE) located in Kansas City, Missouri. The pharmacy provides a variety of services that include antibiotics, cardiac medications, parenteral and enteral nutrition, and a range of specialty medications to address chronic and rare diseases.

**Shift:** Pharmacy is currently open 8-5 M-F. An 8 hour shift will be assigned each day during these times. On-call rotation with other pharmacist. Will include every 4th week and holiday rotation. Support weekend discharges if deemed necessary by pharmacy manager (this is rare).

The Work:

The Home Infusion Pharmacist provides clinical and operational pharmacy services. Core duties of this position include, but are not limited to:

+ Clinical review of medication orders and medical equipment to ensure safe, effective and guideline based medication therapy

+ Monitor therapeutic plans and provide drug information services

+ Communicate and collaborate with interdisciplinary healthcare teams

+ Understand USP 797 sterile compounding standards and implement in daily pharmacy practice

+ Prepare and dispense compounded sterile products (CSP), non-compounded medications and medical equipment

+ Utilize Electronic Medical Record (EMR) to document, analyze and retrieve patient information

+ Supervise supportive personnel

+ Adhere and implement accreditation standards

+ Actively participate in special projects as assigned

+ Assist in all areas of dispensing to meet patient needs

**Requirements:** Missouri and Kansas License, Medication Therapy Services-Missouri Division of Professional Registration

Basic Life Support - American Heart Association or Red Cross, Licensed Pharmacist (KS) - Kansas Board of Pharmacy, Licensed Pharmacist (MO) - Missouri Division of Professional Registration, Medication Therapy Services - Missouri Division of Professional Registration
