    Responsibilities

    + _Provide support and social work services to outpatients if directed by Care Coordination leaders._

    + Participates in performance improvement teams and programs as necessary.

    + Demonstrates behavior that aligns with the Mission and Core Values of the Organization.

    + Responsible for completing required education within established timeframes.

    + Adheres to all hospital policies, standards of practice and Federal or State regulations pertaining to their practice.

    + Performs other duties as assigned.

     

    We are offering a** **$20,000 Sign On Bonus** **to hires who meet the eligibility requirements. We also offer additional pay for participation in our clinical ladder as well as referral bonuses, excellent benefits, and relocation assistance.

    Qualifications

    + Arkansas Licensed Bachelors Social Worker (BSW:AR) OR

    + Arkansas: Licensed Master Social Worker (MSW:AR)

    + At least one year of healthcare experience

    Overview

     

    As a Care Coordinator Social Worker ,your work is crucial in providing support and resources to patients and their families during their time in the hospital and emergency department. You’ll build trust in these interactions by showing compassion, using your critical thinking skills, and remembering that each patient requires a custom care plan tailored to their needs. You will support patients by identifying and removing patient care barriers that will regress their healing. Your talent for creative thinking is vital to your success in case management. It shines as you develop enhanced discharge plans yielding optimal outcomes for each patient.

     

    **CHI St Vincent Hot Springs** is a 280 bed faith-based, non-profit, Magnet Accredited facility providing quality health care to patients and communities from all over the State of Arkansas. As the only level II trauma center in the southwest part of the state, CHI St Vincent Hot Springs is the regional referral center for cardiology, trauma, neuro, ortho, oncology and critical care/pulmonary.     Pay Range

    $21.80 - $31.62 /hour
