Job Description:
This position requires community health workers (CHW) to have a close understanding of the communities they serve. CHWs are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. CHWs creatively leverage patient, family, and community resources to address root causes of patient’s health concerns. CHWs work to increase access to medical, behavioral health, and community services. The goal of this team is to improve the patient’s overall wellbeing through developing trusted relationships with the patient and their family.
Education Qualifications:
Key Responsibilities:
+ Outreach and enroll eligible patients
+ Build trusting relationships with patients and work together to create personalized goals to improve wellbeing
+ Provide tailored support in accessing health care, community connection, and social support
+ Provide referrals for social service resources in the community
+ Advocate for needs of patients and offer social support
+ Communicate with healthcare team about cultural concerns and barriers faced by the patients in the community
Required Qualifications:
+ At least 1 year of experience in customer service, social service, healthcare behavioral health or related field
Preferred Qualifications:
+ Strong written and verbal communication skills
+ Knowledge of local community resources
+ A desire to make a healthy difference in people’s lives
Licensure/Certification Qualifications:
Certification/Licensure Requirements:
+ Community Health Worker certificate or ability to achieve within 18 months of employment or date of hire

**Shift Start/End:** 8:00 am/4:30 pm
**Compensation Range:** $19.08 - $28.62 / hour
**FTE:** 1
