    Adecco Medical & Science is hiring a Field-Based Clinical Care Coordinator RN in the Worcester, MA area.

    • Type: Full-time, contract-to-hire

    • Schedule: Monday to Friday, 8:30 AM – 5:00 PM

    • Location: Worcester, MA (Remote with field-based travel)

    • Pay: $43 to $45/hr

    Responsibilities Include:

    • Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including medical, behavioral, functional, cultural, and socioeconomic factors

    • Assess, plan, and implement individualized care strategies that align with the most appropriate, least restrictive level of care

    • Identify and initiate referrals to social service programs such as financial, psychosocial, community, and state support services

    • Manage the care plan throughout the continuum of care, serving as a single point of contact

    • Collaborate with internal care teams, providers, and community resources to implement and monitor care plans

    • Communicate health-related information to ensure coordinated and timely care

    • Maintain accurate records in compliance with privacy and HIPAA standards

    • Provide high-touch care coordination, often visiting members in their homes, care facilities, or other settings

    Requirements Include:

    • Active and unrestricted Registered Nurse (RN) license in the state of Massachusetts

    • 2+ years of clinical experience

    • Intermediate proficiency with Microsoft Office (Word, Excel, Outlook)

    • Access to a private, secure home workspace and high-speed internet connect

    **Pay Details:** $43.00 to $45.00 per hour
