Chapa-De Indian Health is seeking a passionate and community-rooted Community Health Representative to help connect patients with the care, education, and resources they need to thrive. In this vital outreach role, you'll build trust, provide support, and guide individuals through the healthcare system—ensuring they receive timely services and stay connected to programs that improve their well-being.
Hourly Rate: $21.00 - 24.00, depending on experience!

Community Health Representative
+ Identify patients from their community of focus and inform them of Chapa-De’s services.
+ Establish trusting relationships with patients and their families while providing general support and encouragement.
+ Assist patients in identifying service needs and appropriate resources.
+ Make appropriate in-house and out-of-agency referrals.
+ Provide basic health education, informal counseling, support, and services focused on general health maintenance and promotion.
+ Collaborate and maintain a working relationship with community agencies. Actively network with community agencies, attend community events and represent Chapa-De in the community.
Required Experience:
MINIMUM QUALIFICATIONS:
+ High School diploma or GED certification.
+ Proficiency with Electronic Health Record systems and Microsoft Suite Products (Word, Excel, Teams, etc.).
+ Experience working with Native American populations, including a sensitivity to and familiarity with cultures and customs.
+ Current and in good standing: CDL, vehicle insurance and DMV printout.
