    Position: Skilled Nursing Facility Coordinator (Hybrid/Remote)

    Description

    The Skilled Nursing Facility Coordinator (SNFC) plays a critical role in ensuring seamless transitions of care for patients discharging from skilled nursing facilities (SNFs). This position supports case managers by managing high volumes of documentation, coordinating post-discharge services, and facilitating communication across care teams. The ideal candidate is organized, adaptable, and thrives in a fast-paced, hybrid work environment.

     

    Worksite Location: West Covina, CA

     

    Work Enviroment: Hybrid role (3 days remote, 2 days onsite)

     

    Contract Role: 3-4 months with possibility of extension

     

    Training: 6 weeks

     

    Pay: $25/hr

     

    Schedule: Monday - Friday, no weekends, 8 hr shift, overtime available

    Duties/Responsibilities:

    + Process and prioritize over 100 faxes daily, including discharge orders, medical records, and service requests

    + Coordinate post-discharge services such as transportation, DME, and specialist referrals

    + Review and route clinical documentation (e.g., PASARRs, care plans, diagnosis codes)

    + Support multiple case managers across a designated geographic region

    + Conduct daily patient outreach and manage 150–185 cases per day, including 10–20 discharges

    + Ensure all fax servers are cleared daily and respond to urgent requests in real time

    + Communicate effectively with case managers to clarify unclear or incomplete orders

    + Maintain compliance with HIPAA and internal documentation standards

    Additional Skills & Qualifications

     

    Experience: Minimum 1 year in a healthcare coordination, SNF, or discharge planning role

     

    Preferred Backgrounds: Medical Assistant, Discharge Coordinator, SNF Coordinator

    Technical Skills:

    Familiarity with ICD-9, ICD-10, and CPT codes

     

    Experience with referral and authorization processes

     

    Proficiency in EMR systems and Microsoft Outlook

    Soft Skills:

    Strong multitasking and time management

     

    Clear written and verbal communication

     

    Ability to work independently and collaboratively

     

    Language: Bilingual preferred but not required

    Pay and Benefits

    The pay range for this position is $25.00 - $25.00/hr.
