Community Palliative Care & Geriatric Care Program

Responsibilities

+ Provides nursing support working closely with medical providers,

+ Provides education, ongoing assessment and support to patients and families

+ New patient visit and intake .

+ Triage with patients and family for ll related to Community Palliative Care and Geriatric Memory Assessment Clinic

+ Liaison with PCP offices, LTC facilities, follow up

Required Work Experience:

+ Massachusetts license as an RN and BLS CPR certified.

+ Minimum of 3 years of nursing experience

+ Experience providing clinical care and triage in an outpatient setting

+ Demonstrated interest to work with patients population related to the specialty

+ Excellent verbal and written communication and interpersonal skills.

+ Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians, patients and family members.

Schedule and Location

+ Location - 21 Dwight Road , Longmeadow

+ Full Time 40 hours - No Weekends No Holidays

+ Monday- Friday 8-4:30, or 8:30-5 or 9-5:30

+ Sign On Bonus
