Job Title: RN MDS Coordinator

Job Type: Full-time

Shift: Day

Pay Range: $42.00 – $57.50/hour (Final offer based on skills, experience, and professional background)

As an RN MDS Coordinator, you’ll play a pivotal role in resident assessment and care planning, making a lasting difference in the lives of seniors while advancing your nursing career in a collaborative, resident-centered environment.

As a Registered Nurse MDS Coordinator, you will:

+ Lead the Assessment Process: Oversee and coordinate the completion of the Minimum Data Set (MDS) and resident assessments to ensure accuracy and compliance with federal and state regulations.

+ Drive Quality Outcomes: Collaborate with interdisciplinary teams to develop individualized care plans that enhance resident well-being and quality of life.

+ Ensure Regulatory Compliance: Maintain up-to-date knowledge of CMS, COMAR, and Office of Health Care Quality standards, ensuring all documentation and processes meet regulatory requirements.

+ Support Team Excellence: Educate and mentor nursing staff on MDS processes, care planning, and best practices for documentation.

+ Facilitate Communication: Serve as a key liaison between clinical staff, residents, families, and leadership to support strategic goals and exceptional resident outcomes.

Requirements

+ Active RN license

+ 2+ years of experience in a clinical setting, preferably in long-term care

+ Knowledge of Medicare, Medicaid, and CMS regulations and requirements

+ Previous MDS experience is preferred
