Overview

St. Luke’s Health System is seeking a Telehealth Patient Management Specialist to join our Ambulatory Care team. In this role, you will help support patients with chronic conditions. You will be an essential part of a collaborative team focused on improving access to care, reducing hospital visits, and empowering patients to manage their health more effectively.

We currently have both full-time and part-time onsite positions available.

Position Details:

+ **Shift:** 3x12-hour shifts (7:00 AM – 7:00 PM)

+ **Schedule:** Rotating weekends and holidays required

+ **Department:** Telehealth – Ambulatory Remote Patient Monitoring

+ **Pay Range:** $17.32 – $19. plus, per hour, depending on experience

What You’ll Do:

+ Onboard new patients into the remote patient management program

+ Monitor and manage patients living with chronic conditions

+ Assist patients in adhering to care plans and maintaining compliance with program guidelines

+ Provide basic technical support for remote monitoring equipment

+ Perform scheduling functions, similar to a clinic setting

+ Utilize medical terminology and clinical knowledge to support communication between patients and care teams

Qualifications:

High school diploma or equivalent required

Preferred Qualifications:

- Experience as a Certified Nursing Assistant (CNA), Unit Clerk, or in a similar role

- Familiarity with medical terminology

- Background in a healthcare setting, ideally ambulatory or telehealth
