Job Description
Chase Memorial Nursing Home and Rehab is actively looking to fill the Admissions and Marketing Coordinator position. The purpose of this position is to assume authority, responsibility, and accountability for the delivery of admission and community education services. Collaborates and coordinates in an efficient and economic sound process to maintain resident census based on facility goals. Develops and implements a marketing program, acts as a liaison between the facility and community, including health care entities to communicate facility goals and approaches to attain and maintain quality of care. The Admissions and Marketing Coordinator should be a self-starter, takes initiative, is a motivated/driven professional, experienced with outreach, be able to build relationships , is goal oriented and have the ability to work as a team and individually.

Essential Job Requirements
Associates degree in any Health or Human Services field OR Sales and Marketing
Preferred:
Demonstrated knowledge of long-term care, marketing principles, federal and state regulations applicable to long-term care environment, and understanding of third-party reimbursement.
Able to work in fast paced environment, good organization skills, goal oriented, and have excellent customer service skills.
Essential Job Functions:
Admissions
• Maintains admission guidelines by writing, updating, and recommending changes to admission criteria, policies and procedures based on applicable standards, which may include guiding the team in the discharge planning process to evaluate and effectively meet length of stay determinants.
• Markets programs by preparing and providing informational brochures; writing and placing advertisements; answering questions; conducting tours; network with community healthcare entities
• Obtains applicant information by requesting completed applications and medical information; verifying and clarifying information; interviewing patients and family members; explaining admission criteria.
• Screens patients by comparing patient's condition to admission criteria; evaluating and accepting or rejecting patients; referring patients and family to other programs and institutions.
• Works with medical, nursing, therapy, social work, and accounting staff to ensure appropriate patient placement. Confirms that all insurance benefits coverage meets standards of admission as dictated by policy.
• Ensures admission and financial responsibility forms are completed on day of admission, coordinating and arranging physical, social, emotional, and support service requirements
• Covers for discharge planning coordinator by overseeing discharge process to ensure all physical, social, emotional, and physical services are in place for a safe discharge
• Works with the IDT to help resolve patient/family dissatisfactions by investigating concerns, recommending changes in service policies and procedures.
• Prepares admissions, d/c, transfer reports by collecting, analyzing, and summarizing data and trends.
• Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks.
• Prepares and reviews with the IDT critical data that directly involves quality and regulations, including taking an active role in the implementation of INTERACT
Marketing
+ Gathers, analyzes, and presents marketing research data that promotes the development of a marketing program for the facility that reflects the need of health care entities, insurance programs, and other community organizations.
+ Participates in community civic meetings, boards, and fundraisers on a regular basis to ensure representation from Chase and to keep abreast of changes in the market
+ Makes daily contact (in person is ideal) to referral sources, spending the majority of the day outside of the facility in sales/marketing to gain admissions to the facility as needed
+ Evaluates resident and family satisfaction through survey’s and leads the IDT in improving any deficient practice verbalized
+ Leads marketing committee for fund raisers
+ Works collaboratively with internal and external partners to develop media (e.g. mass advertising, promotional programs, social media, direct marketing and event strategies).
