    Responsibilities:

    GENERAL PURPOSE

    The Community Navigator serves as a bridge between the community and the health care, and social service systems. Responsibilities may include helping individuals, families, groups and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information, providing informal counseling, support, and follow-up, translating and interpreting for clients and health care/social service providers, identifying critical resources, navigating individuals through health care services and systems, and promoting community and individual health. All efforts of this position are to be accomplished with limited direction from the Senior Regional Outreach Coordinator (SROC), following the principles of diversity, equity, and inclusion to support the four aims of the COE office.

    ESSENTIAL FUNCTIONS

    Service Navigation

    + Review health intake assessments collected during community outreach and assist in connecting community members with appropriate cancer screenings via phone calls, text messaging, emails and/or mail.

    + Report adequate follow up from health assessment referrals to SROC and COE leadership team.

    + Improve navigation workflow between community members and URMC cancer screenings and risk reduction programs.

    + Work with other navigators to ensure continuity of cancer care.

    Community Partnerships and Outreach

    + Participate in outreach activities as needed.

    + Research, seek out, and identify opportunities to collaborate with individuals and community partners such as schools, churches, barbershops, municipalities, and other community-based organizations to develop, coordinate and deliver methods for cancer education in culturally appropriate ways.

    + Develop, maintain, and foster critical connections and working relationships with community agencies, organizations, small businesses, and other groups interested in cancer education.

    + Organize, develop, prepare, and deliver educational materials for programs based on cancer awareness, prevention, risk reduction and early detection strategies.

    + Identifies opportunities to link WCI/URMC resources to community-identified needs and promotes the work of URMC in community health as it pertains to cancer awareness, prevention, risk reduction and early detection and make recommendations.

    + Assist in monitoring and the evaluation of the short-term and long-term goals of the COE.

    Tobacco Quit Center Screening and Navigation

    + Assist the Wilmot Tobacco Treatment specialist in:

    + Conducting referral follow up and initial consultations.

    + Tracking cessation status follow up.

    + Documentation and data reporting.

    Data Entry and Management

    + Assist in data input and management in online databases, including Redcap and Excel and contribute to data analysis.

    + Perform data quality assurance procedures, analyze information, and follow up and resolve discrepancies.

    + Prepares, reviews, and analyzes data for report summaries as needed.

    MINIMUM EDUCATION & EXPERIENCE

    + Bachelor’s degree and 2 years of relevant experience required

    + or equivalent combination of education and experience required.

    KNOWLEDGE, SKILLS AND ABILITIES

    + Bilingual English/Spanish speaker

    + Excellent interpersonal, and communication skills (written and oral)

    + Ability to interact effectively with a wide range of diverse populations from varying cultural/ethnic, socio-economic and educational levels. Broad understanding of cultural competence and cultural humility

    + Willingness to work flexible hours, including some evenings and weekends

    LICENSES AND CERTIFICATIONS

    + DL NUMBER – Driver License, Valid and in State and reliable transportation or ability to move between locations during shift upon hire
