What you will do:
The Resident Advisor Program was established for Seniors living in Residential Independent Housing facilities. The goal of the program is to help enable residents to effectively age in place longer and to maintain their highest level of Independence. Resident Advisors (caseworker) inform seniors and their families about all services that are available to make their lives easier and to meet their current and ever-changing needs.
Candidate must be engaging, detail oriented, excellent at multi-tasking and excel at customer service and satisfaction.
Responsibilities
+ The caseworker will orient new residents to the community and complete a file of life form for their refrigerator.
+ Maintain case files documenting resident profile, emergency numbers, service needs and resolution of needs. Maintain a file with copies of Advanced directives and copies of medical cards.
+ Meets with residents to listen to concerns, discuss goals and engage in problem solving. May develop individualized plan of care.
+ Will maintain a directory of community resources to include doctors, PT, transportation services, move in and move out coordinators, durable medical equipment companies, Health Alerts and companies providing home care services. The case worker will then connect residents and families with these services as the need arises and follow up to ensure that the resident is satisfied with the service
+ Will educate residents and families regarding higher levels of care when needed. Will guide them through the application process. Caseworker should have working knowledge of Long Term Care insurance and be able to assist families in understanding their benefits.
+ Will provide Health/Wellness speaker quarterly to present current information related to senior health issues
+ Will organize and facilitate flu and covid booster clinics, podiatry services, medication days with pharmacy students and drug take back days.
+ Will work with families as they move out from the community providing them with resources to assist them in the move.
What you will need:
+ Bachelor's degree in social work, sociology, psychology, counseling or related field is required.
+ 2-5 years previous experience in a geriatric or health care setting.
+ Working knowledge of community resources, including but not limited to: APS, caregiver support, substance use disorders and behavioral health services and Medicaid/Medicare regulations.
+ Knowledge of issues and regulations related to end of life care/advance directives/voluntary and involuntary
+ Ability to communicate effectively both written and verbally.
Pay Range: $23.40 - $33.69
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
