    Health Information Clerk

    Job Summary

    Performs clerical functions related to ongoing department operations and the coordination of interdepartmental and intradepartmental service and communications. Performs a variety of functions related to processing, maintenance, and security of patient health records.

    Job Requirements

    Minimum Education: High school diploma/ or equivalent required. Associate degree preferred in relevant field.

     

    Required Skills: Previous hospital or Health Information Management experience preferred, with strong interpersonal skills. Previous experience with CPSI, Athena and Medhost electronic health records (EHR) beneficial.

     

    + Demonstrated knowledge of medical terminology required

    + Demonstrated knowledge of chart assembly and background knowledge of chart analysis is desired

    + Thorough knowledge of dictation/transcription equipment and usage

    + Demonstrated knowledge of basic clerical duties and skills: scanning, typing, pulling records, filing, photocopying, answering telephones, faxing.

    + Demonstrated knowledge and proficiency of electronic medical record systems and computer applications

    + Demonstrated knowledge of essential elements for report formats

    + Demonstrated understanding of concurrent chart flow and patient types

    + Demonstrated understanding of patient flow from admit to discharge required

    + Demonstrated knowledge of release of confidential health information, terminal digit filing and basic medical office practices.

    + Reliable attendance and punctuality, including observing appropriate break times and extending a scheduled shift when necessary to meet patient demands.

     

    Functional Demands

     

    PHYSICAL EFFORT AND ENVIRONMENTS: Work involves sitting at a computer terminal or scanner 80% of the time. Walking, bending, stooping and minimal lifting is required. Vision and hearing acuteness is required.

     

    Sedentary: Occasionally lifts up to 25 pounds independently. Sits most of the time. Stands or walks briefly.

     

    EXPOSURE RISK Exposure Category III: Employee not reasonably expected to be at risk for exposure to blood borne pathogens.

    • Develops and implements appropriate quality improvements for the department.

    + Develops quality initiative projects and the methods necessary for initiating, testing, data collection, analysis, and reporting the effectiveness of the project. s

    + Establishes indicators to monitor departmental service in response to complaints, identified problems, variances, sentinel events, or new services as well as patient outcome-oriented indicators for high volume and/or high-risk services.

    + Reports all Quality Improvement activities, measurements and results according to the established reporting structure.

    + Participates in the annual review of the Quality Improvement/Patient Safety Program.

    + Participates as a member of a functional and/or process improvement team as appropriate to position or requested.

    + Provides Quality Improvement orientation, training, and continuing education for all departmental staff.

    + Involves all departmental staff members in the Quality Improvement Program’s plans and processes.

    + Is accountable for identification and mitigation of risks and hazards within the department.
