    Position Summary

    The supervisor for the OhioRISE Care Management Associate Team is responsible for oversight and management of team processes including the organization and development of high performing teams.

    Position Responsibilities:

    • Reinforces clinical philosophy, programs, policies, and procedures.

    • Computer literacy and experience with Teams, Outlook, Microsoft Excel, Word, and other web-based applications.

    • Communicates strategic plan and specific tactics to meet plan.

    • Ability to evaluate and interpret data, identify areas of improvement, and focus on interventions to improve outcomes.

    • Serve as a model to team members regarding practice standards, quality of interventions, problem resolution and critical thinking.

    • Ensures implementation of tactics to meet strategic direction for cost and quality outcomes.

    • Assist in the support, development, and oversight of all communication and documentation standards required for Care Coordination.

    • Ability to multi-task on various projects, utilizing multiple systems and ensure priority deadlines are met.

    • Possess ability to work independently and can take initiative on identifying interim/permanent solutions to daily or large-scale problems.

    • Support and establish a positive work environment that promotes teamwork, acknowledges team contributions, solicits input, and offers personal assistance when needed.

    • Ability to maintain flexibility and adjust to changing needs within health plan needs that may require ongoing re-prioritization of tasks.

    • Identifies opportunities to implement best practice approaches and introduce innovations to better improve outcomes.

    • May develop/assist in creation and/or review new training content for immediate team and/or collaborate/deliver inter-departmental training sessions as needed.

    • Participate in new staff training and onboarding.

    • Accountable for the day-to-day management of assigned team for appropriate implementation and adherence with established practices, policies, and procedures.

    • Works closely with functional area managers to ensure consistency in interventions supporting our plan sponsors.

    • Develop, initiate, monitor, and communicate performance expectations.

    • Participate in the recruitment and hiring process for staff using clearly defined requirements in terms of education, experience, technical and performance skills.

    • Assesses developmental needs and collaborates with others to identify and implement action plans that support the development of high performing teams.

    • Consistently demonstrates the ability to serve as a model change agent and lead change efforts.

    • Accountable for maintaining compliance with policies and procedures and implements them at the employee level.

    Required Qualifications

    • Must reside in state of Ohio.

    • Bachelor’s Degree

    • Prior Relevant Work Experience

    • Essential Qualifications

    • Willing and able to participate in up to 10% of travel for any as needed health plan meetings.

    Preferred Qualifications

    + **Proven ability to think critically and independently,** demonstrating sound judgment, logical reasoning, and practical problem solving in a fast paced environment.

    + **Strong sense of accountability,** with a track record of retaining and applying information with minimal supervision

    + **Experience in leading teams or mentoring others,** in health care, operations, or case management setting preferred, but not required if the candidate demonstrates a high level of teachability and leadership potential

    + **Demonstrated ability to manage multiple projects or initiatives simultaneously** , with effective prioritization, follow through, and adaptability to shifting priorities

    + **Excellent communication and interpersonal skills** , with the ability to collaborate across departments and build strong relationships with internal and external partners

    + **Strategic thinker** , capable of identifying gaps and opportunities, proposing creative solutions, and contributing to team goals and process improvement initiatives

    + Proficient in navigating systems and data to inform decisions, track outcomes, and ensure compliance with policies and protocols

    + Comfortable receiving and giving feedback, and committed to personal growth and team development

    Pay Range

    The typical pay range for this role is:

    $43,888.00 - $76,500.00

     

    This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
