We are looking for a unique and talented Home Health Physical Therapist to deliver skilled interventions, advocate for the patient, and treat them like a real person while doing so! Act as the patient’s partner and friend as they restore their previous level of function. It's your job to give them the tools, knowledge, and motivation to return to their best. Multiple coverage areas available now, so please ask. ***This position will primarily cover cities east of 77, From Shaker Heights and south to Sagamore Hills*** Responsibilities * DELIVER KINDNESS FIRST * Identify and meet patients’ goals and needs * Offer skilled treatments that help improve clients’ motion and mobility * Reduce the need for medications and provide alternatives to surgery * Develop care plans using a variety of treatment techniques * Create skilled programs tailored to patients’ specific needs * Provide quality, personalized and evidence-based care and proven interventions * Motivate patients during treatment in order to help them function optimally * Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination * Consult and practice with other health professionals * Evaluate effects and monitor and communicate progress * Document patient care services Skills * Proven work experience as a physical therapist * Intensive education and clinical expertise * Ability to manage patients with different types of personalities * Current knowledge of treatment practices * Drive to continually learn and grow * Professional behavior * Excellent interpersonal communication skills * Valid license to practice physical therapy * Graduate degree or greater in physical therapy     Job Type: Full-time/Part-time/PRN Salary: $55.00 - $95.00 per visit depending on the visit type.  Benefits: * 401(k) * Flexible schedule * Health/Dental/Vision insurance reimbursement  * Paid time off * Referral program * Retirement plan   Medical specialties: * Geriatrics * Home Health * Physical & Rehabilitation Medicine   Schedule: * Day shift * Monday to Friday * Weekend availability but not required   License/Certification: * CPR Certification  * Physical Therapy License    Work Location: On the road
