We are seeking community coordinators for our client located in Newark, NJ. Job description is below. Apply today for immediate consideration!
Title: Community Outreach Coordinator
Pay rate: $25/hr
Hours: Monday through Friday 8:30 am - 4:30 pm
Duration: 12 months with possible extension or hire
Duties:
+ Provide educational information and resources as needed to the patient population such as medical assistance, social services information, patient wellness resources, low cost housing or shelter information.
+ Respond with sensitivity to the cultural and socio-economic issues of the consumer
+ Link patients to appropriate clinical and social services, including but not limited to health insurance, housing, and disability benefits.
+ Provide reminder calls for all upcoming appointments as needed.
+ Maintain appropriate consumer records in time as mandated by Center policy.
+ Maintains conduct in the process of performing duties that are consistently characterized by a professional demeanor.
+ Identify and assist patients in overcoming barriers to recovery goals and participation in medical or mental health services.
+ Collaborate with physicians, social workers, nurses, and other team members serving patients with in providing services
+ Performs other related duties as assigned.
Requirements:
1. Bachelor’s degree in Public Health or HS Diploma with experience in community outreach activities especially with Opioid Use Disorder initiatives.
2. Strong communication skills, dependable and reliable
3. Proficient in MS Office Suite
4. Professional demeanor
5. Must have transportation in order to attend local events as needed.
**Pay Details:** $25.00 per hour
